Who We Are:
North Alabama Shoals Hospital is a 198-bed general acute care facility boasting spacious, private rooms for all patients, a 24-hour emergency department, medical and oncology services; medical/surgical inpatient units; and short-stay surgery units. Shoals also operates the region’s only in-patient acute rehabilitation facility, the J. W. Sommer Rehabilitation Unit, and the Shoals Senior Care Center, the Shoals’ only psychiatric unit dedicated to our senior adults.
Position Summary:
Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems. Acts as Director of Social Work for Inpatient Behavioral Health accreditation purposes only.
- Education: Master’s degree Required
- Licenses: Licensed Master's Social Worker (LMSW)
- Certifications: Basic Life Support Health Care Provider (BLS-HCP); Crisis Intervention Training (within 90 days hire/transfer)
- 2 years related experience
Required Skills
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
Essential Functions
- Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
- Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
- Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
- Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
- Performs Medicaid screenings as indicated for NHP.
- Assists patients with Safety Net applications when needed.
Director of Social Work functions:
Monitors and evaluates the quality and appropriateness of social services on the inpatient behavioral health units in accordance with accepted standards of practice and established policies and procedures.
Conducts a bio-psycho-social assessment of the patient and participates in the treatment planning process, with ongoing reassessment and evaluation.
Formulates individualized measurable goals for the patient.
Participates in discharge planning, arranging for follow up care, continuity of care and coordination with outside agencies.
Uses various modalities such as group therapy, individual therapy to treat the patient and assist families
Participates in utilization management activities as needed to assure proper payment for services.
Completes required documentation within required time frames.
Non-Essential Functions
Assist with orientation and training of personnel and students.
Participates and attends meetings as required and/ or assigned.
Performs other related duties as assigned or requested.
