Social Worker- LMSW- Home Health

Choice Health at Home is seeking a compassionate and dedicated PRN Licensed Master's Social Worker (LMSW) to join our Home Health team. In this role, you'll provide psychosocial assessments, counseling, education, and community resource coordination to help patients and their families navigate healthcare challenges while remaining safely at home. What You'll Do

As a PRN Home Health Licensed Master's Social Worker, you will:

• Complete comprehensive psychosocial assessments to identify patients' emotional, social, financial, environmental, and community resource needs.
• Develop and implement individualized care plans in collaboration with physicians, nurses, therapists, and the interdisciplinary care team.
• Provide counseling and emotional support to patients and caregivers as they adjust to illness, disability, or changes in health status.
• Assist patients and families with community resource referrals, financial assistance programs, advance directives, long-term planning, and discharge planning.
• Educate patients and caregivers on available services to promote independence and improve quality of life.
• Participate in interdisciplinary case conferences to coordinate high-quality patient care.
• Maintain accurate, timely documentation in accordance with agency policies and Medicare Conditions of Participation.
• Advocate for patients while promoting dignity, respect, and patient-centered care.

Qualifications

Required:

• Master's Degree in Social Work (MSW) from an accredited program
• Current Licensed Master's Social Worker (LMSW) license in the state of practice
• One or more years of Home Health experience preferred
• Current AHA CPR Certification
• Valid driver's license, reliable transportation, and current auto insurance

Preferred:

• HomeCare HomeBase (HCHB) experience
