SUMMARY:

The Licensed Social Worker is responsible for implementing resident services for residents consistent with regulations and established best practices. The Social Worker provides support through resident/family advocacy, ongoing assessment and evaluation, service coordination and care planning. The primary focus of this position is the quality of life of residents to assist them in achieving their optimal psychosocial, emotional and behavioral well-being. This position reports to the Campus Director of Social Services and be covering our long term care facility.

JOB SPECIFICATIONS:

Education, Experience and Certifications

  • Bachelor’s degree in Social Work, Gerontology, or related field required.  An equivalent combination of professional experience and education considered at discretion of management.  
  • Licensed by the MN Board of Social Work and maintain unrestricted licensure.
  • One or more years of social work experience preferred, preferably in a long-term care environment.
  • Transitional Care Unit (TCU):  One or more years of social work experience preferred, in a long-term care setting.

Compensation: An individual's pay rate within the posted range may be determined by various factors, including skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization prioritizes pay equity and considers internal team equity when making any offer. Hiring at the maximum of the range is not typical. Shift and/or other differentials may be available to increase your pay rate for certain shifts or work.
