Be a part of a world-class academic healthcare system at UChicago Medicine as a Social Worker II in our Multispecialty clinic at our Crown Point location.

 

In this role, you will be a member of the multidisciplinary team who provides consultation, assessment, and interventions for patients and families in need of emotional, social, or psychological support.

 

Essential Job Functions:

  • Conduct comprehensive psychosocial assessments to identify patient and family needs and develop appropriate care plans
  • Provide counseling, crisis intervention, emotional support, and resource guidance to patients and families
  • Coordinate discharge planning and transitions of care, including referrals to community resources, extended care facilities, hospice, and support services
  • Collaborate with physicians, nurses, case managers, and other interdisciplinary team members to ensure quality patient care
  • Advocate for patients and families by connecting them with financial, social, behavioral health, and community resources
  • Educate patients and families on psychosocial issues, treatment plans, available services, and healthcare resources
  • Assess and respond to concerns related to abuse, neglect, safety, mental health, substance use, and other psychosocial risks in accordance with regulatory requirements
  • Facilitate support groups, family meetings, and care conferences as appropriate
  • Maintain timely, accurate documentation in the electronic medical record
  • Participate in multidisciplinary rounds, professional development activities, and departmental initiatives to support quality patient outcomes

 

Required Qualifications:

  • Master’s degree in social work from a School of Social Work accredited by the Council on Social Work Education
  • Indiana Social Work License or must be eligible for licensure and secure it during probation period
  • Previous clinical social work experience in a health care setting of at least one year
  • Requires knowledge of the principles of growth and development over the lifespan

Compensation & Benefits Overview

 

UChicago Medicine is committed to transparency in compensation and benefits.  The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

 

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
