The Pines at Davidson is hiring a Social Worker to ensure top-notch care for our residents in our assisted living and long-term care settings. The health center social worker plays a vital role in providing social work services to residents. This position involves assessing the social and emotional needs of residents and their families, developing care plans, and coordinating with interdisciplinary teams to ensure residents' well-being and quality of life. The social worker also provides counseling to residents and their families, advocates for residents’ rights, and facilitates community resources and referrals.

Who you are



Bachelor’s or master’s Degree in social work or related field. 


North Carolina State Licensure or certification as a social worker. 


Previous experience in a skilled nursing facility or long-term care setting preferred. 


Knowledge of social work principles, ethics, and practices. 


Effective communication, interpersonal and organizational skills. 


Ability to work effectively in a team environment and collaborate with interdisciplinary teams. 


Compassion, empathy, and a commitment to advocating for residents’ well-being.



ESSENTIAL JOB FUNCTIONS: 



Conduct assessments of residents’ social and emotional needs and develop individualized care plans. 


Provide counseling and emotional support to residents and their families to address issues such as adjustment to illness, grief, placement, and end-of-life care. 


Collaborate with interdisciplinary teams, including medical staff, therapists, nurses, and caregivers, to ensure holistic care for residents. 


Advocate for residents’ rights and provide information and education to residents, families and team members on resident’s rights and responsibilities. 


Assist residents in accessing community resources, such as financial assistance, housing, and social services as needed. 


Maintain accurate and confidential documentation of social work assessments, psychosocial assessments, interventions, and progress notes. 


Participate in care conferences and function as the care conference coordinator for resident care conferences. 


Participate in team meetings, leadership meetings, routine in-services, and training to stay informed on resident care and social work practices. 


Stay up to date on regulations and laws related to senior services and healthcare social work practices. 


Acts as health center formal grievance officer, assisting with coordinating grievance process and ensuring timely, appropriate follow-up. 


May participate in other duties as assigned. 



PHYSICAL REQUIREMENTS (AS DETERMINED USING WORKSHEET): (Include Vision, Language, Hearing, Range of Motion, Lifting Capacity, Sitting/Standing, etc.) 


Extended periods of sitting, walking and standing.   
Some driving required.   
Occasionally required to lift up to 10 lbs.  
Repetitive use of hands includes:  simple grasping of less than 10 lbs. frequently; fine manipulation using key board and writing frequently. 
Sensory requirements:  Write, keyboarding, speak on phone and in person, speak to large groups, read computer monitor, hear spoken questions and comments in person, on phone and in large groups. 
Work performed in an indoor setting with occasional walking outdoors to various campus locations.
