Social Worker, Case Manager

The Social Work, Case Manager has the responsibility of performing clinical assessments, utilization review and discharge planning for all patients admitted to the acute inpatient rehabilitation facility at Stanford Hospital.

Major Responsibilities include

Acts as liaison between clinical team, patients, family and insurance providers through the rehabilitation program process.

Facilitates the planning, coordination and discharge for a specified patient population in the inpatient rehab facility with the multidisciplinary team.

Provides ongoing support and expertise through comprehensive assessment, care planning, implementation and evaluation of individual patient needs.

Acts as a content expert for social work, case management and hospital personnel re: acute rehabilitation unit criteria for admission.

Completes insurance authorization process by providing an initial plan of care and securing insurance authorization for services rendered and coordinating services following discharge from the acute rehab unit.

Maintain up to date and knowledgeable about CMS regulations regarding acute inpatient rehabilitation and participates in annual UDS Training and competency.
