What You Will Do

  • You will engage directly with patients to assess their unique priorities, provide meaningful support, and maintain accurate, thorough documentation of your case activity.
  • Working closely with providers, team managers, and external agencies, you will ensure every patient’s treatment plan is comprehensive, compliant, and well-supported by their family and social network.
  • You’ll be a connector—building and nurturing relationships with community health, welfare, and social agencies to create creative, effective solutions for our patients’ needs.
  • You will guide families through complex systems, making successful referrals to community resources that provide the practical assistance they need most.
  • You will play a key role in the patient journey by supporting discharge planning, concurrent reviews, and the coordination of essential post-discharge services like home care.

What You Will Need

Licensure / Certification / Registration:

  • Social Worker credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.

Education:

  • High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.

Work Experience:

  • 3 years of experience preferred.
