About the Social Worker Role

As our Social Worker, you'll partner with physicians, nurses, and case managers to plan safe, timely discharges for medically complex patients. You'll be a key advocate for patients and families — helping them understand their options, connect with the right resources, and move confidently into the next stage of care.

What You'll Do

  • Support patients and families through discharge planning, financial concerns, and the emotional impact of acute illness
  • Provide supportive counseling as patients adjust to illness and address broader biopsychosocial needs
  • Coordinate with external agencies to ensure a smooth, well-informed transition out of the hospital

What is Required to be a Social Worker @ Landmark Hospitals

  • Bachelor's degree in Social Work required; Master's degree preferred
  • Active, unrestricted Missouri LMSW or LCSW preferred
  • 5+ years of hospital-based social work experience
  • Strong working knowledge of community resources, referral agencies, and healthcare systems
  • Sharp analytical and critical-thinking skills, with the ability to manage multiple complex cases at once
  • A collaborative, empathetic approach and excellent communication skills across a multidisciplinary team
