Overview

The Behavioral Wellness Center at Girard is seeking a Social Worker to join our Behavioral Health team in Philadelphia.

This position provides direct social work and clinical support to individuals receiving treatment for mental health conditions, substance use disorders, addiction, and co occurring behavioral health needs. The Social Worker serves as an active member of the multidisciplinary treatment team and supports patients throughout treatment, from admission through discharge and aftercare planning.

Key areas of responsibility include psychosocial assessment, treatment planning, individual counseling, group facilitation, discharge planning, community resource coordination, benefits and financial assistance, family support, and coordination with outside agencies and providers.

We are open to recent graduates with a Bachelor’s or Master’s degree in Social Work who are looking to begin their career in behavioral health. Previous professional Social Work experience is preferred but not required.

This opportunity may be a strong fit for Social Work graduates with internship, field placement, practicum, volunteer, or professional exposure to behavioral health, mental health, addiction recovery, case management, discharge planning, community resources, residential treatment, or related social services settings.

Responsibilities

• Develop and maintain professional relationships with referring agencies

• Meet with prospective clients and participate in preliminary interviews with program leadership

• Collect clinical records and information to support admission evaluations

• Assist with admissions procedures, including treatment consents, financial agreements, and other required documentation

• Assist with the development of psychosocial histories and evaluations during the early stages of treatment

• Participate in the development of initial, comprehensive, and updated treatment plans

• Lead social work groups as assigned by the Program Director or Clinical Supervisor

• Provide individual counseling to clients as required

• Participate in multidisciplinary treatment team meetings

• Assist clients with maintaining medical coverage and accessing financial assistance

• Maintain communication with outside providers, community agencies, and support systems

• Assist clients in developing realistic discharge and aftercare plans, including housing and therapeutic supports

• Maintain accurate and timely documentation within patient records and the Electronic Health Record

• Communicate important clinical information to other members of the treatment team in a timely manner

• Complete treatment planning, counseling interventions, and documentation based on individual clinical needs

• Perform duties in accordance with organizational requirements and applicable professional ethical standards

• Perform other duties as assigned

Qualifications

Education & Experience

Bachelor’s degree in Social Work (BSW) required

Master’s degree in Social Work (MSW) preferred

Recent graduates with a Bachelor’s or Master’s degree in Social Work are encouraged to apply

• Previous professional Social Work experience preferred but not required

• Internship, field placement, practicum, volunteer, or professional experience in behavioral health, mental health, addiction recovery, case management, discharge planning, community based services, or related social work settings preferred

• Bilingual capabilities are a plus

Work Environment

This is a fully onsite position within a behavioral health treatment environment involving direct patient contact.

The Social Worker works closely with patients, families, clinical staff, physicians, outside referral sources, and other members of the multidisciplinary treatment team.

The position requires the ability to manage deadlines, maintain accurate documentation, work within electronic systems, and respond appropriately to crisis situations.

The role may involve extended periods of sitting, standing, walking, and working within patient care and treatment areas. Employees are expected to follow hospital health, infection control, ethics, confidentiality, and safety requirements.
