Fox Subacute at Mechanicsburg ( the leader in long term care for residents with challenging respiratory issues) is seeking a Full Time Social Worker (SW) to develop and implement the program and procedures related to social work.

Responsible for demonstrating the Mission for Fox Subacute and to champion the best care possible including quality of life for all residents in accordance with federal, state and local standards and regulations and Fox Subacute polices/procedures.

You are expected to align with our Mission and Values in partnership with the organization, collaborating with leadership to push for our goals to support the continued success of Fox Subacute.

  • Leads by example by modeling the highest level of integrity, honesty and fairness
  • Bachelor’s Degree in social work or similar field
  • Previous leadership experience in the social work field
  • Instructional and mentor skills
  • Able to read, write and speak the English language clearly
  • Must be a minimum of 18 years of age
  • Previous experience in Long Term Care
  • Ability to lead, inspire and discipline staff members
  • A desire to serve and care for residents
  • Is able to follow written and oral instructions
  • Ability to handle multiple priorities
  • Possess written and verbal skills for effective communication and documentation
  • Competent in organization, time management skills
  • Demonstrates good judgment, problem solving and decision making skills
  • Ability to work weekends, evenings and flexible hours
  • Ability to react and remain calm in difficult situations and be considerate
  • Essential Duties: Resident Care Duties
  • Plans, organizes, implements and evaluates the Social Service Programs and activities
  • Coordinates with administration and clinical leadership
  • Develops a rapport with other departments and community organizations
  • Develops and maintains Social Services Policies and Procedures
  • Assists in planning and implementing community discharges
  • Assists in Medicaid and Insurance issues
  • Interviews, documents and fulfills MDS requirements
  • Assists families with admission providing information and forms
  • Coordinates practices to maintaining resident property
  • Demonstrates knowledge of federal, state and other applicable regulations
  • Assists the DCS in the direction of the interdisciplinary team including Care Conference, Care Plans, CQI and other related practices
  • Supports communication on all levels
  • Is proactive in maintaining a neat and orderly facility
