The Lead Social Worker will be responsible for the design, implementation, and management of a social work program to assess and address the social needs of patients served within a faith-based, integrated primary care setting. 

 KEY RESPONSIBILITIES

1.  Plan, develop, implement and evaluate all aspects of the Social Work program.

2.  Collaborate with the Medical, Dental, and Behavioral Health Chairs as well as other key clinical personnel to identify the greatest social needs of Christ Community’s various clinics.

3.  Create and maintain a list of current community resources for use by Social Workers, trainees, staff, and providers to address the various social needs of patients.

4.  Develop and maintain relationships with key leadership personnel of various community partners in order to expand patient access to resources and to facilitate health centers’ connections within the community.

5.  Develop, implement, and monitor SDoH screening process for all CCHS patients, including referral and loop closure.

6.  Maintain, foster and further develop relationships with local academic institutions who train Social Work students to aid in carrying out the provision of resource management.

7.  Select and provide supervision to Social Work students from local institutions who will be serving in the various primary care clinics providing direct assessment of social needs of patients and resource management.

8.  Assure that Social Work students training within the organization are provided with a well-rounded supervision plan during their rotation within the organization and that trainees meet all requirements of their specific academic programs.

9.  Responsible for collaborating with Chair and HR for the recruitment, hiring, and onboarding of all clinic embedded social workers, focused on SDoH and Care Management.

10. Partner will billing, MCOs and IT to ensure implementation of billing model for SDoH for financial sustainability in collaboration with BH Chair.

11. Collaborate with BH Chair and Clinical Leads for BH Care Management, both related to PCMH requirements as well as insured mental health patients, including billing.  Track and monitor progress as well as compliance with program for the social workers.

12. Together with the Chair of BH and the social work team, maintain education for all providers and clinical staff in SDoH and Care Management.

13. Collaborate with Chair of Behavioral Health quality metrics to assess and monitor outcomes of Social Work implementation to determine if additional Social Work staff are needed to ensure integration of social work services as outlined in the strategic priorities as determined by the Chief Executive Officer and Leadership team. 

14. Performs other duties as required.

 POSITION REQUIREMENTS

Education:  Master’s degree in Social Work preferred, if only a batchelor's in social work must have a minimum five years’ experience. 

Experience:  1) Two (2) years of post-graduate experience preferably in a hospital or other primary healthcare setting, 2) experience serving a culturally and socioeconomically diverse, urban population, 3) significant experience providing supervision for Social Work trainees or other Social Work employees, 4) prior management and program development experience preferred

 Licenses or Certifications:  LMSW or LCSW preferred, if TN licenses batchelor’s level social work, must pursue license within six months
