The Dialysis Social Work Administrative Assistant supports the licensed social worker in providing psychosocial services to patients receiving dialysis treatment. This role assists with care coordination, resource linkage, patient support, and administrative duties to enhance patient well-being and treatment adherence.

 

Schedule: Part-time, 30 hours/week @ three 10-hour days, shifts starting at 7am

 

Compensation: Pay range $24-$27 per hour, depending on qualifications and experience

Responsibilities

What You Can Expect

 

Patient Support & Engagement

  • Assist in providing emotional support to patients adjusting to dialysis treatment (e.g., hemodialysis or peritoneal dialysis).
  • Reinforce education on treatment adherence, coping strategies, and lifestyle adjustments.
  • Observe and report changes in patient mood, behavior, or psychosocial status to the social worker.

Care Coordination

  • Help coordinate patient services including transportation, housing, financial assistance, and community resources.
  • Assist SW with preparing and distributing a variety of documents and information - monthly psychosocial assessment package, patient contact & insurance updates, welcome package for new patients, information for QAPI meetings, Patient Quality of Life Surveys, updating transplant list, etc.
  • Input various patient information in Darwin - patient consents, history, healthcare proxy, etc.
  • Review patient Grievance and Complaint binder daily.
  • Coordinate and assist SW with Lobby days and quarterly workshops for patients.

Administrative Support

  • Maintain accurate and timely documentation in patient records per facility policy.
  • Assist with scheduling appointments, follow-ups, and patient outreach.
  • Help track patient compliance and attendance issues (e.g., missed or shortened treatments).

Patient Advocacy

  • Advocate for patient needs under the supervision of the licensed social worker.
  • Assist patients with navigating insurance, benefits (Medicare/Medicaid), and financial concerns.

Team Collaboration

  • Work closely with interdisciplinary team members including nurses, dietitians, and physicians.
  • Participate in care planning meetings and contribute relevant observations, as needed.

Qualifications

  • Ability to maintain confidentiality (HIPAA compliance)
  • Empathy and cultural sensitivity
  • Basic knowledge of community resources
Education/Training:
  • High school diploma required.
  • Associate’s or Bachelor’s degree in Social Work, Human Services, or related field preferred.
  • Prior experience in healthcare, dialysis, or social services preferred.
  • Experience working with chronically ill or vulnerable populations is a plus.
