Responsible for the case management services offered to families through the More Than A Job NC (MTAJ-NC) program. Manages and monitors assigned caseload of clients seeking social service assistance and/or participating in a life management program; supervises the day-to-day operations and staff providing social services and shelter services to homeless clients; provides social services offered by The Salvation Army; works with clients to develop goals and evaluates clients progress by conducting routine meetings with client and/or staff; serves as advocate for client in order to acquire services and skills that will enable them to move toward self-sufficiency.

 

Key Responsibilities

Reviews, monitors, and implements program policy and procedures; administers program in compliance with budget constraints.

Interviews applicants to determine eligibility for program enrollment based on established guidelines and requirements.

Conducts needs assessments, obtains pertinent information, and establish program goals for eligible clients; refers applicants to other agencies if not appropriate for program.

Completes intake and case plans for client; instructs clients in completing necessary paperwork including goals and/or budget forms.

Prepares and maintains case records, notes, and logs on all assigned clients in a timely fashion; ensures the accuracy and completeness of the same.

Completes discharge plans of clients exiting the program; transfers file contents to non-active file.

Conduct a written assessment on all potential applicants and their families within 72 hours of contact including screening for serious personal safety and mental health issues.

Develop a case plan with the participant based on the primary goals of housing stability, increased income/financial resources and skills, and self-determination within one week of intake which includes short-term and long-term goals with objectives. Update case plan as needed.

Schedule regular meeting times with participants to develop and review goals and objectives.

Plans, directs, and supervises the work of assigned staff; trains and instructs employee in the proper methods and procedures; monitors work in progress and upon completion to ensure accuracy, completeness, and compliance with established policies and procedures; conducts performance evaluations and provides hiring/firing recommendations.

Ensures that the provision of emergency financial assistance services are responsive to current professional case management standards; ensures that appropriate documentation of client services and support materials for financial resources offered to clients meets contractual standards; ensures representation is managed at community poverty and financial assistance collaborations.

Maintain working relationships with community agencies to provide comprehensive services for participants and to stay abreast of current trends and resources available.

 
QualificationsEducation and Experience

Bachelor’s degree from an accredited college or university in Social Work or a related field;

and

Three (3) years performing progressively responsible social service case management work, preferably with homeless clients;

or

Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
