We are seeking a skilled and detail-oriented Social Worker Manager to join our team. This role is essential in supervising, guiding, and supporting social work staff, overseeing the delivery of social services, and ensuring compliance with regulatory standards. The Social Worker Manager is responsible for maintaining the quality, reliability, and effectiveness of social services, and supporting the smooth operation of our facility.

Starting Rate: $78,874;Β higher compensation possibly available based on experience.

Duties & Responsibilities:Β 

  • Directly supervise, mentor, and evaluate nursing home medical social workers and assistants at the Cedarbrook Allentown Facility and Brookview Apartments.
  • Clinical & Administrative Oversight over social services related assessment and care planning processes for long-term care and short-term rehabilitation, including participation in related meetings.
  • Compliance with all regulatory standards and employment of QAPI related processes to ensure quality care.
  • Interdisciplinary and collaborative approach with other departments and other government agencies to ensure residents receive appropriate services and supports.
  • Maintain consistent, professional, and expedient communication with Cedarbrook residents and their families and act as an advocate for residents to receive needed care, services, supports. and other benefits.
  • Ensure appropriate documentation processes occur for admissions, discharge/transfers, and other supportive services.

Qualifications:

  • Bachelor's degree in social work or related Field required; Pennsylvania licensure (LSW/LCSW) or MSW highly preferred.
  • Minimum 2 years' experience in a health care or long-term care setting, with supervisory experience or lead experience in social services.
  • Standard business hours with on-call duties for emergencies and ability to adjust schedule for emergencies and other important events. Ability to drive to Cedarbrook Fountain Hill and other locations for external events on an as-needed basis with valid PA Driver's License.
  • Strong knowledge of PA and federal nursing home regulations.
  • Excellent organizational, communication, and interpersonal skills.
  • Ability to work collaboratively with interdisciplinary teams and external agencies.
  • Proficiency in Microsoft Office products and electronic medical records and documentation systems. PointClickCare Experience is highly preferred.
  • Or any equivalent combination of acceptable training and experience which has provided the knowledge, skills and abilities cited above as determined by Cedarbrook.
