Visalia Post Acute is seeking a compassionate, organized, and experienced Full-Time Social Services Director to support the emotional, social, and psychosocial well-being of our residents.

This position works closely with residents, families, nursing staff, therapy professionals, community agencies, and the interdisciplinary care team. The ideal candidate is a strong advocate who can manage sensitive situations with professionalism, objectivity, persistence, and confidentiality.

A minimum of a bachelor’s degree is required for this position.

Essential Duties and Responsibilities

  • Assess residents’ social, emotional, psychological, and family needs.
  • Develop appropriate goals and courses of action in collaboration with residents and their families.
  • Provide medically related social services that help residents achieve or maintain their highest practicable level of physical, mental, and psychosocial well-being.
  • Assist with developing, reviewing, and updating social-services care plans.
  • Help residents achieve the highest possible level of independence, self-care, and overall well-being.
  • Provide emotional support to residents and families coping with illness, disability, loss, grief, or changes in condition.
  • Counsel residents and family members regarding adjustment, care decisions, and available resources.
  • Assist residents and families with healthcare decisions and advance-care planning as appropriate.
  • Participate in interdisciplinary care conferences and communicate resident needs to the care team.
  • Coordinate discharge planning with residents, families, facility departments, community agencies, equipment providers, and outside service organizations.
  • Arrange referrals and connect residents with appropriate community, health, and social-service resources.
  • Assist with scheduling outpatient appointments and on-site ancillary services, including optometry, podiatry, dentistry, and psychiatric services.
  • Assist residents in obtaining appropriate prosthetics, assistive devices, and other necessary resources.
  • Maintain accurate, timely, and confidential social-services documentation.
  • Assist with personal shopping and other resident-support needs as appropriate.
  • Help inventory and track residents’ personal belongings.
  • Coordinate responses to reports of missing, lost, or stolen resident property.
  • Develop and maintain strong relationships with facility departments and outside community agencies.
  • Provide staff education and in-service training as requested.
  • Advocate for resident rights, dignity, preferences, and quality of life.
  • Maintain the confidentiality of all resident health and personal information.
  • Perform additional duties as assigned.

Minimum Qualifications

  • Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or a closely related field required.
  • At least two years of supervised social-work experience in a healthcare setting working directly with individuals required.
  • Master of Social Work degree preferred.
  • Skilled nursing, long-term care, rehabilitation, or geriatric experience strongly preferred.
  • Knowledge of resident rights, discharge planning, care planning, community resources, and psychosocial assessment.
  • Experience with PointClickCare or another electronic health-record system preferred.
  • Strong counseling, crisis-management, advocacy, and problem-solving abilities.
  • Excellent written and verbal communication skills.
  • Ability to manage sensitive information with discretion and confidentiality.
  • Strong organization, documentation, planning, and time-management skills.
  • Ability to work effectively with residents, families, staff, healthcare providers, and community agencies.
  • Compassionate, professional, proactive, and resident-centered approach.

Supervisory Responsibilities

This position has no direct supervisory responsibilities.

Physical Requirements

This position frequently requires standing, walking, reaching, pushing, pulling, speaking, and hearing, with occasional sitting, climbing, balancing, stooping, kneeling, or crouching. The employee must be able to lift or move up to 25 pounds.
