 Job Summary:
 
The job of Coordinator of Social Services is a licensed Social Worker and will be in keeping with the general philosophy of all services provided at Holy Angels Residential Facility.  Job responsibilities should be carried out in accordance with the recommended procedures to be followed as dictated by State and Federal Licensing and Long-Term Care and ICF-DD regulations. The coordinator serves as a facilitator in providing the necessary services to the residents of Holy Angels on a daily basis.

  
Specific Skills and Responsibilities
  
·         Assists in the admissions process of each resident.  Field/screens in-coming inquiries regarding placement, provides information and 
          tours to potential residents, reviews all applications for admissions, and acts as liaison throughout the admissions process.
·         Coordinates with records clerk to set up a Master File on all residents, and updates files when information changes.  
·         Maintains annual staffing list, informs appropriate people of the current staffing schedule, and maintains quarterly updates of 
          residents' family contacts.
·         Reviews and updates information in the Social Services section of the Individual Program Plan.
·         Answers routine questions regarding welfare and concerns of residents, their parents/guardians.
·         Acts as liaison with Holy Angels' consulting attorney regarding questions on residents’ legal status or rights under the law or legal 
          status and obligations in its relationship to residents, parents and guardians.  Attend meetings and in-services as needed.
·         Coordinates with Admission Director and Resident Services Director to safeguard residents' rights, investigates allegations of abuse 
          and neglect, and is mindful of confidentiality regarding resident medical records and family information.
·         Liaison with the federal, state and local agencies as necessary to assist with placement of suitable candidates under their care or 
          supervision.
·         Performs any other duties assigned by the Resident Services and Admission Directors.

 
Qualifications:
 
·         Minimum 5 years’ experience in Social Work
·         Must maintain a current and valid driver’s license
 
Education: 
·         Master’s degree in social work Required
·         Current Social Work License
 
Work Environment:
 
·         Must be able to work with individuals with developmental disabilities
 
Physical Demands:
 
·         Walk, stand, sit, push, pull, stoop, kneel, crouch, reach, use hands and lift up to 50lbs, 2/3’s of the time
