JOB DESCRIPTION
SOCIAL SERVICES SPECIALIST
JOB SUMMARY:
Works closely with and liaises between Case Management, Work Program, Security, and Reception departments. Assists with visitors to the facility including patient visitors, regulators, advocates, etc. Also will assist with Human Resources and Staff Development as needed with candidates and new hires for orientation and onboarding processes. Duties include phone triage, addressing requests for services and possible crisis intervention, answers calls with regard to hospital services, bed availability, general inquiries; offers resources when applicable, verifies insurance, utilizes our electronic medical records system, completes intakes and paperwork relevant to intakes. Reports to the Director of Social Services.
ESSENTIAL FUNCTIONS:
Provides clinical screening/assessment for potential and admitted patients and or Employee Health/ Human Resources.
Provides information and referral services for internal and external customers and/or candidates/employees.
Interviews incoming patient or representative and enters data into EMR; assures admission criteria is met
Presents clinical information to a member of medical staff for approval/denial of admissions
Explains hospital regulations, patient rights and all admissions documentation to patient/representative
Records/collects necessary personal information and secures required signatures
Verifies patient information (custody arrangement, HCS, demographic data, etc)
Verifies insurance benefits/payment information/collects co-pays as appropriate
Performs duties and records all information concisely/accurately, and completely.
Identifies and utilizes appropriate channels of communication
Provides crisis intervention as needed
ADDITIONAL DUTIES
Ensures adequate continuing education credits to maintain licensure if required
Assists Candidates for hire with application process.
Assists with Reception duties and provides coverage as needed.
Assists Security with patient visitation as needed.
Assists Staff Development with Education, Orientation and Competencies as needed.
Provides patient and family education as needed
Attends/completes staff development activities as required
Attends monthly staff meetings
Responsible for knowledge/review of Admission/HCHI policies
Other related duties as assigned
QualificationsQUALIFICATIONS:
To perform this job successfully, and individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or ability required. Education and Experience:
BS/BA Mental Health or Behavioral Health field-psychology, social work, criminal justice, nursing etc.
Knowledge of community resources and previous experience in mental health facility preferred.
Clinical expertise is preferred.
Language Skills:
Ability to read and interpret instructions, documents, and follow procedures.
Ability to write routine reports and speak effectively with patients and their families, employees, and public.
Reasoning Ability:
Ability to solve practical problems and deal with a variety of variables as needed.
Ability to interpret a variety or instructions furnished in written, oral, diagram, or schedule form.
Other Skills and Abilities:
Computer literate, working knowledge of DOS, Windows, CMHC and other applications.
Working knowledge of the DSM-IV and/or DSM-V
Outstanding interpersonal and interviewing and assessment skills.
