Job Summary:
Works with Social Service Manager in coordination with Care Management and Medical Staff and Nursing Service in providing social services to hospital patients and their families. The position requires knowledge of the age and developmentally specific needs of the child through geriatric population. Functions within the Care Management Department.
Essential Job Duties:
- Discharge Planning/Assessment and Referral – Performs assessments and coordinates planning to assure patient's healthcare needs are met on discharge. Coordinates appropriate referrals to community agencies and facilities.
- Documentation – Documents on all cases in a timely and accurate fashion according to Social Service and Hospital policy.
- Prioritizes as necessary in order to meet important deadlines.
- PI - Participates in P.I. projects as requested by Social Services Manager.
Secondary Job Duties That May be Reassigned:
- Financial—Counsels patients and families regarding disability, Medicare, Medicaid and other Financial resources as available. Refers to Claim Aid as appropriate
- CPS/APS/Dialysis/Adoption/Surragacy - Counsels and refers in coordination with medical and nursing staff all cases of child abuse and neglect, adult abuse and neglect, dialysis arrangements and adoption/surragacy coordination.
Qualifications
Job Specifications:
- Education: Bachelor's degree in Social Work or related field.
- Experience: Two years of hospital Social Work experience preferred
