The Social Care Network (SCN) Program Manager oversees the implementation, coordination, compliance, and day-to-day operations of the Ali Forney Center’s participation in New York State’s Medicaid 1115 Waiver Social Care Network initiative. The Program Manager ensures that staff are trained, supported, and operating in compliance with New York State Department of Health (NYSDOH), Office of Health Insurance Programs (OHIP), SCN, and Managed Care Organization (MCO) requirements.

This role serves as the internal operational lead for AFC’s SCN work across screening, navigation, enhanced HRSN services, nutrition supports, housing-related supports, referrals, billing coordination, and data reporting. The Program Manager will work closely with frontline staff, finance, compliance, program leadership, and external SCN partners to ensure high-quality service delivery and reimbursement readiness.

 

Key Responsibilities


Coordinate implementation and ongoing operations of AFC’s SCN program across Manhattan, Brooklyn, and Queens.
Ensure compliance with SCN policies, NYHER (New York Health Equity Reform) 1115 Waiver guidance, SCN Operations Manual requirements, and Medicaid documentation standards.
Supervise and support SCN staff delivering screenings, navigation, nutrition, housing, and care coordination services.
Develop workflows to ensure timely completion of screenings, eligibility assessments, social care plans, referrals, and service documentation within the SCN IT Platform.
Monitor program performance metrics, billing readiness, referral completion, and service delivery timelines.
Coordinate onboarding and required trainings for SCN staff, including trauma-informed care, cultural competency, HIPAA, data privacy, mandated reporting, and SCN platform training.
Serve as AFC’s point person with Public Health Solutions and SCN operational partners.
Support quality assurance activities, audits, reporting, and corrective action plans as needed.
Ensure proper collection and maintenance of member consent and confidential health information in compliance with HIPAA and NYS regulations.
Collaborate with finance and billing staff to support reimbursement tracking and issue resolution.
Assist in developing policies, SOPs, training materials, and internal compliance tools related to SCN services.
Monitor staffing capacity and help identify operational gaps and program improvement opportunities.


 
Qualifications and Competencies 


Bachelor’s degree required
Minimum 3–5 years of experience in Medicaid-funded programs, care coordination, social services, housing, or healthcare operations..
Knowledge of health-related social needs (HRSN), trauma-informed care, LGBTQ+ youth services, and housing instability preferred.
Strong project management, compliance, and operational oversight skills.
Experience supervising staff and coordinating multidisciplinary teams.
Strong written, organizational, and communication skills.
Ability to manage multiple priorities in a fast-paced environment.


Preferred but not required


Master’s degree in Social Work, Public Health, Health Administration, or related field preferred.
Experience with New York State Medicaid systems, managed care, or 1115 Waiver initiatives strongly preferred
