Position Description:
Case Manager: Help Men Build a Real Plan for Life After Incarceration
180 Degrees believes every person deserves a real shot at a fresh start. Clifton Place serves adult men transitioning from incarceration and navigating community supervision. Many of the men we serve are carrying the weight of long-term system involvement, trauma, substance use, housing instability, and disrupted access to healthcare and employment. For many of these men, lasting change starts with a solid plan and a Case Manager who believes they can get there.
As the Case Manager at Clifton Place, you provide intensive, ongoing case management to residents living in a sober, supportive residential setting. From entry to exit, you're the steady guide helping residents remain compliant with legal requirements, access community resources, and build a realistic plan for housing, income, and ongoing support. Using trauma-informed, person-centered, and accountability-based practices, you work alongside residents to reduce barriers, strengthen skills, and support long-term stability while maintaining safety, structure, and program expectations.

What you'll actually do:
- Provide intensive, ongoing case management to residents throughout their time at Clifton Place
- Support residents in remaining compliant with court-ordered conditions and legal requirements
- Help residents access community resources related to housing, employment, healthcare, and more
- Build a realistic exit plan with each resident covering housing, income, and ongoing support
- Apply trauma-informed, person-centered, and accountability-based practices to reduce barriers and strengthen skills
- Maintain safety, structure, and program expectations while supporting long-term stability
- Build consistent, trusting relationships with residents navigating stabilization and reentry

This role is for you if:

This is a full-time position, 40 hours per week, onsite. No direct reports, but direct impact on every resident's path forward.
Qualifications:
- Minimum age: 21.
- Bachelor’s degree in human services, psychology, social services, social work, or a related field and two years of related case management experience in criminal justice, community re-entry, or homelessness; case management experience may be substituted in lieu of education on a year-for-year basis.
- Two years of experience in the nonprofit sector required.
- Valid driver’s license and insurance; dependable transportation for traveling to community partners and events; ability to drive Company vehicle without restrictions.
- Ability to clear a Department of Human Services background screening.
Essential Competencies:
- Three years of related case management experience in criminal justice, community re-entry, or homelessness preferred.
- Four years of experience in the nonprofit sector preferred.
- Strong foundation in trauma-responsive and youth-centered practice.
- Ability to build rapport with youth and families while maintaining professional boundaries.
- Skilled in care coordination, advocacy, and collaborative problem-solving.
- Understanding of confidentiality, data practices, and ethical responsibilities.
- Strong written and verbal communication skills and documentation skills.
- Excellent organization, time management, and attention to detail.
- Ability to operate effectively in fast-paced, complex environments.
- Proficiency with computer systems and databases.

While performing the duties of this job, the employee is regularly required to use a computer. The employee is frequently required walk, stand, and go up and down stairs. Regular computer use. Combination of seated work and movement within the shelter and community. Occasional evening, weekend, or holiday hours, including on-call responsibilities. Travel to community partners and appointments as required.
Work Environment:
This role moves with the rhythm of a program that never closes. 180 Degrees operates 24/7/365. Crisis doesn't run on a 9-to-5, and neither do we. Case Managers participate in the on-call rotation, and flexibility to work extended hours for events and special projects is part of showing up fully for this work. This is an on-site position at our recently renovated St. Paul location which serves female-identifying youth.
