Service Plan Coordinator, QCP
The Arc Of Union CountySummary: The Arc of Union County, Inc. is a leading provider of Person-Centered services to individuals with intellectual and developmental disabilities through a range of programs. Under the guidance of the Director, Quality, Compliance & Planning, the Service Plan Coordinator is a professional responsible for developing, implementing, and monitoring individualized service plans for service recipients, often in social, healthcare, or community-based agencies. Their function is central to ensuring that participants receive coordinated, person-centered services that meet their needs, preferences, and goals. The SPC ensures that service recipients receive timely, individualized, and coordinated services by combining person-centered planning, interagency collaboration, advocacy, monitoring, and administrative compliance. The incumbent promotes service recipient independence, well-being, and the effective use of community and agency-based resources.
Qualifications:
- A bachelor’s degree in fields such as Social Work, Special Education, Human Services, Rehabilitation, Behavioral Sciences, or Health-Related Sciences is required, with state-specific training in Intellectual/Developmental Disabilities (IDD) or disability services preferred.
- Candidates must have at least 5 years of experience in social services or working with individuals with IDD.
- Additionally, obtaining certification as an ASQ (American Society for Quality) or CQA (Certified Quality Auditor) is required within six months of hiring.
- The applicant should be knowledgeable about DDD (Division of Developmental Disabilities) day habilitation services, DVR (Division of Vocational Rehabilitation) services, and residential services under NJ 10.44A standards. They must also understand family support services in accordance with DCF (Department of Children and Families) and CARF (Commission on Accreditation of Rehabilitation Facilities) standards for individuals with intellectual and/or developmental disabilities, as well as state and federal eligibility requirements for these services.
- Experience with Medicaid waiver programs or other community-based support systems is highly preferred. Previous involvement in person-centered planning and care coordination.

The physical demands for this position must be met by an employee to successfully perform the essential functions of the job. While performing the duties of this job, the employee may be regularly required to maintain a stationary position (standing and sitting), move/traverse, kneel, bend, reach with hands and arms for extended periods of time, twist, carry, lift up to 35 pounds, Agility with wrist and finger dexterity to access, key and sort electronic information or file documents, ability to view computer and/or electronic device for extended period and other physical duties as directed by your supervisor and/or Director.
The essential Cognitive Functions of this position may include the ability to analyze, compute, hear, and repeat information, take notes, read documents, generate written information, independently problem solve, tolerate interruptions, maintain positive public relations, organize, verbally communicate, and converse with a diverse group of people, among others.
