What You'll Do
As the Service Coordinator, you will serve as a trusted advocate and resource for residents living in St. Joseph Place HUD 202 Senior Housing community.
Key responsibilities include:
- Conduct assessments to identify residents' social, health, and support needs.
- Provide case management, resource referrals, and service coordination.
- Connect residents with community services such as transportation, meal programs, counseling, wellness programs, legal resources, personal assistance, and healthcare services.
- Build and maintain strong relationships with local agencies, service providers, and community partners.
- Advocate for residents and help them navigate applications, services, and available benefits.
- Develop individualized service plans and monitor ongoing support services.
- Help residents strengthen relationships with family, friends, neighbors, and community resources.
- Coordinate educational programs and training that support successful aging in place.
- Recruit and develop volunteer opportunities that enhance resident engagement and well-being.
- Partner with staff to promote a resident-centered environment that values dignity, independence, and quality of life.

Qualifications
Required
- Bachelor's degree in Social Work or a related field OR
- Minimum of five (5) years of experience in social services.
Preferred
- At least one (1) year of experience working with older adults or in a geriatric setting.
