Action Inc. is seeking two Service Advocates to join our Supportive Services team. This is meaningful, relationship-centered work with individuals and families navigating housing instability, critical transitions, and other barriers to stability. Service Advocates help people identify goals, connect with resources, build skills, and move toward greater stability and independence.

Who You'll Serve

Service Advocates work with youth, young adults, adults, and families who may be navigating housing instability, critical transitions, or other barriers to stability. Assignments may vary based on program and community needs, so flexibility and the ability to work effectively with people across age groups are important to this role.

What You'll Do

• Build respectful, trusting relationships and provide person-centered, trauma-informed case management.

• Assess individual needs and barriers, then develop, monitor, and update goal plans with participants.

• Help people navigate housing, benefits, employment, education, life skills, and other community supports based on individual      needs and program eligibility.

• Conduct outreach, connect eligible individuals with services, and coordinate referrals with community partners.

• Work collaboratively with Action Inc. staff and community providers to strengthen service coordination and participant outcomes.

• Maintain timely, accurate case notes, participant files, data entry, statistics, and documentation required by funding sources.

• Apply program guidelines and funding requirements thoughtfully while helping participants identify practical pathways forward.

• Maintain professional boundaries, confidentiality, and a calm, respectful approach during challenging situations.

Requirements

Who We're Looking For

We are looking for someone who is compassionate, resourceful, organized, and able to meet people where they are without judgment. The right person can build rapport while maintaining professional boundaries, communicate about sensitive issues, problem-solve with participants rather than for them, and work effectively as part of a collaborative team.

• Bachelor's degree in a human services field and two years of case management or related experience preferred; other combinations of education and experience will be considered.

• Experience working with youth, adults, families, people experiencing homelessness, or other populations facing barriers to stability is valued.

• Knowledge of community resources, trauma-informed practice, housing stabilization, workforce preparation, life-skill development, or related approaches is helpful.

• Strong written communication and documentation skills, including clear and accurate case notes.

• Ability to travel throughout the service area; valid driver's license and ability to meet agency insurability requirements.
