Responsibilities

 

We are looking for a professional who is responsible for meeting the psycho-social needs of patients, who acts as a referral source and liaison between patients, their families and community agencies, in order to assist the patient in optimally transitioning from one level of care to another. Additional responsibilities include serving as a liaison with community agencies, providing consultations to medical-surgical patients and screenings of Emergency Department psychiatric patients as needed. Additionally, providing coordination of the Social Work staff, and supporting their performance relative to social work assessment, discharge planning, completion of psychiatric screens, communication skills (written and verbal), participation in team conferences and unit-based rounds, group facilitation and specific daily assignments.

The compensation range for the role is $41.76-$53.33 hourly.

 

Garnet Health System provides a compensation range to comply with the New York State law on Salary Transparency in Job Advertisements. Qualifications

Minimum Education: Master’s degree in Social Work

 

Minimum Experience:

  • Minimum of 3 years’ hands on experience in social work required, with 5 or more years’ preferred. Experience in the acute care/Emergency Department setting preferred.
  • Demonstrated assessment skills.
  • Ability to utilize problem solving theories and techniques, combined with flexibility and creativity, in working with a diverse population.
  • Must possess time management skills, highly developed verbal and written skills, problem solving capacity, critical thinking, and good organizational skills.
  • Requires knowledge of community resources, third party payers, and regulatory agencies.
  • Competency on computer is preferred.

Required Certification/Registration:

  • Currently licensed and registered in NY State to practice as a social worker (LMSW or LCSW).
  • BLS certification required upon hire, and maintained for the duration of employment.
  • Must attend hospital crisis program within 3 months of hire or transfer and annually thereafter.

Physical Requirements: Position will primarily be medium to light work requiring lifting and/or carrying objects weighing up to 50 pounds.  Bending, twisting, stooping, kneeling, and reaching are required.  Position requires 20% sitting and 80% standing or walking.  Expressing or exchanging ideas by spoken and written word is required.

  • Environmental Demands and Exposure to Hazards: Works in a clean, well-lighted, heated or ventilated facility.  No routine exposure to hazards. Position may have occasional exposure to blood, body substances, infectious materials, chemical hazards, vibration, radiation, slippery surfaces, or noise that may result from performance of duties.
  • Physical Demands: Demonstrates physical and functional ability to perform full anatomical range of motion to accomplish tasks. Evidence of visual and aural acuity and finger and hand dexterity to operate computer and office equipment. Can withstand long periods of sitting, standing and/or constant walking. Ability to lift 50 lbs.
  • Mental Demands: Ability to foster collaborative relationships, to work well under pressure, to organize and synthesize new information, and prioritize tasks. Possesses critical thinking, analytical skills and flexibility. Ability to multi-task. Required detailed attention to work in an environment where interruptions cannot be controlled. Demonstrates sensitivity to customer needs and expectations. May be subject to irregular hours including evenings or potentially weekends to participate in operational and community events as necessary.
