 RESPONSIBILITIES 

  • Conduct participant intakes and assessments, ensuring timely and accurate data entry into the Participant Tracking System (PTS) funder database and Salesforce.
  • Provide clients with internal and external referrals as needed 
  • Perform client advocacy and conduct monthly follow-ups to monitor service delivery, assess client progress, and ensure continued engagement with recommended services.
  • Establish and monitor quality assurance processes to ensure program goals align with agency standards and contractual requirements
  • Monitor the program’s contractual compliance and meet with the contract manager during site visits
  • Document client progress and all interactions with or on behalf of clients in the database
  • Participate in community outreach events as scheduled 
  • Meet monthly with the Program Director/Associate Vice President for the exchange of ideas, program updates, and written feedback.
  • Submit monthly reports to supervisor and funding source.
  • Perform additional duties as assigned. 

 

QUALIFICATIONS 

  • Bachelor’s Degree with 1 year experience in benefits and gov’t entitlements
  • Bilingual both verbal and written preferred 
  • Proficiency in modern business communications including personal computers, electronic mail, voicemail, and copier equipment 
  • Proficiency in virtual platforms is preferred
  • Develop a customized service plan with clients, outlining action steps and responsibilities.
  • Maintain up to date case files and database entries, as well as regular contact with clients via office visits, telephone contact and home visits.
  • Coordinate client referrals for other needed services
  • Other duties as assigned

 

PHYSICAL REQUIREMENTS 

  • Ability to use a computer for prolonged periods
  • Ability to occasionally lift and/or move up to 10 pounds
  • Ability to stand, walk, or sit for long periods of time
  • Ability to bend and retrieve objects and/or documents
  • Ability to commute within the Bronx
