Position Summary:
The Care Coordinator provides care coordination services for individuals and families served through both the School-Based Program and the Psychosis Early Action Team (PEAT). This position supports clients by connecting them with the community resources, addressing barriers to care, coordinating services, and promoting engagement with behavioral health and social support systems. The Care Coordinator works collaboratively with clinicians, families, schools, and community partners to improve client outcomes and ensure continuity of care.
Key Responsibilities:
- Support the Clinical Coordinator with client registration, documentation, and other administrative functions related to the PEAT program.
- Receive referrals from school-based clinicians for clients and families requiring care coordination, with a focus on addressing unmet basic needs.
- Identify and connect clients with appropriate community resources and services, while coordinating referrals, follow-up, and ongoing engagement.
- Build trusting, supportive relationships with clients and their families through consistent communication and advocacy.
- Collaborate with clients and families to establish individualized goals and provide ongoing monitoring and support.
- Conduct outreach and engagement activities through in-person visits, telephone calls, mail, email and other communication methods.
- Complete initial and ongoing needs assessments to identify barriers to care, including transportation, housing and food insecurity, access to community resources, social supports, language needs, literacy, and cultural preferences.
- Provide culturally responsive education, information, and support to clients and families regarding available health and behavioral health services.
- Assist families in securing transportation for appointments, evaluations, and other healthcare services.
- Participate in community meetings, health fairs, and outreach events to increase awareness of available services and strengthen community partnerships.
- Collaborate with multidisciplinary teams, including School-Based Program staff and the PEAT team, to coordinate care and support positive client outcomes.
- Attend weekly team meetings and contribute to care planning, case reviews, and program development.
Qualifications:
Required:
- Bachelor's degree in Psychology, Social Work, Public Health, Human Services, or a related field.
- Experience working with children, adolescents, young adults, and families.
- Strong organizational skills with excellent time management and attention to detail.
- Ability to work collaboratively as part of a multidisciplinary team while managing an independent caseload.
Preferred:
- Experience working with individuals experiencing chronic mental health and/or medical conditions.
- Knowledge of Department of Mental Health (DMH) services and referral processes.
- Strong organizational skills with excellent time management and attention to detail.
- Excellent written, verbal and interpersonal communication skills.
- Bilingual skills are preferred.

Summary of Benefits:
- Salary Range: $22.00-$24.00 hourly
Based on years of experience, level of education, specialty and licensure/certifications. Additional Compensation for advanced education for qualifying positions.
