POSITION:                        Rapid Re-Housing Stability Case Manager
REPORTS TO:                  CWF Program Manager
STATUS:                           Full Time- Hourly
 
OVERVIEW:  Under the direct supervision of the Rapid Re-Housing Program Manager, will provide case management to individuals experiencing homelessness in the Rapid Rehousing Program. Using a housing first approach, the SHIP RRH Case Manager follows established low barrier guidelines to create a safe and stable environment for individuals entering the program. The SHIP RRH Specialist requires a high degree of professionalism, commitment and adaptability in order to effectively uphold coalition values and maintain an environment of dignity, respect and cooperation.
PRIMARY DUTIES:
 
  • Maintain regular contact with clients throughout the case management phase. And maintain case notes and document in HMIS.
  • Work collaboratively with clients using a strength-based approach to develop their Housing Stability Plans to address issues that may impede access to housing.
  • Assist clients with the entire housing process to include; completing applications, paying application fees, negotiating manageable and appropriate lease agreements with landlords.
  • Assist with move-in to unit to ensure the client has necessary household items and basic understanding of the unit.
  • Mediate with landlords regarding tenant issues.
  • De-escalate crises.
  • Make appropriate and time-limited services and supports available clients to allow them to stabilize quickly in permanent housing, or provide Rapid Rehousing services
  • Provide or assist clients with connections to resources that help them improve their safety and well-being and achieve their long-term goals.
  • Monitor clients housing stability and be available to support the resolution of crises, at a minimum during the period that rental assistance is provided.
  • Maintain and submit documents required for monthly reporting.
  • Implement a referral process to access shelter resources for unsheltered singles/families if needed.
  • Role model effective team behaviors.
  • Demonstrate effective communication skills in building relationships with all staff and clients.
  • Treat all clients, volunteers and staff with respect and dignity.
  • Maintain strict confidentiality of all information.
  • Adhere to Coalition policies and procedures.
  • Meet with newly assigned clients within one business day.
  • Maintain the Case Manager Log regularly
  • Evaluate and adjust IPPs as action steps are completed and new goals/steps arise.
  • Empower clients to become involved in their own housing planning and goal setting.
  • Complete case notes on every face to face/telephone/email contact with client or collateral contact within 24 hours.
  • Gather all documents needed for the client and/or household (ID, Birth Certificate, Social Security Card, etc.) and upload onto client’s profile in HMIS. Assist client with documents acquisition if they do not have them at point of intake.
  • Regularly schedule appointments with clients to ensure that they are on target with their IPP.
  • Maintain and submit data required for monthly reporting.
  • Distribute referrals as needed and check with referral agencies for follow up information when needed.  Referrals may include resources for housing, childcare, tangible goods, financial assistance, health care, employment programs, and behavioral health among others.
  • Work in collaboration with program and all other agency staff to facilitate a team environment.
  • Maintain client files in good order.
  • Present cases at case staff meetings for input from other staff members and commit to group decisions.
 
QUALIFICATIONS:
  • Candidates must possess an Associate’s Degree or equivalent number of college credits in a social services field.   A Bachelor’s Degree is preferred.  Degrees from other disciplines will be accepted given the candidate provides evidence of prior experience that would allow them to competently perform the primary duties listed below. Candidate should have at least 3 years of documented case management experience.
  • Individuals that have been working in the field, but do not possess a degree will be considered if they have five or more years of documented case management experience.
  • Homeless services, case management, mental health, permanent supportive housing and rapid rehousing experience preferred.
  • Flexible work schedule to include some evenings and/or weekends. Will be expected to meet clients at times that are convenient for the client and outside of regular work schedule.
  • Must be proficient in written and verbal communication.
  • The ability to effectively serve a very diverse population with aptitude in the areas of conflict resolution, critical thinking and problem solving.
