Resource Associate

Job Description

Assists in identifying and utilizing community resources for patients and their families. Liaisons with various agencies regarding social work issues and maintains all pertinent records.

Job Responsibility

• * Assists in identifying and using community resources for patients and/or their families including visits to patient's homes.
• * Assists in establishing liaison and communications with other agencies including visits to agencies.
• * Maintains a variety of records and reports relating to social work activity
• Performs related duties, as required.

Job Qualification

• Associate of Arts/Science Degree or equivalent college coursework, in Behavioral and/or Social Sciences, required.
• Demonstrated interest in the field of Social Welfare via educational, previous employment, fieldwork, volunteer work, and/or life experience, required.
