Physical Requirements: Lifting: Ability to lift and transfer people supported, lift 50lbs, walking, climbing, bending, reaching, kneeling, squatting, twisting. Ability to carry out SCIP RR techniques is required.
Hours of Work : Full-time, 40 hours per week. Weekend, evening and holiday work is required for Residential positions.
Job Summary: The Residential Habilitation Specialist works directly with individuals who have intellectual or developmental disabilities and their support teams. They ensure that the individual's treatment and habilitation needs are effective and work closely with the clinical and nursing department as a liaison for the residential program. The Residential Habilitation Specialist will provide compliance and quality checks to ensure Direct Support Professional (DSP) responsibilities have been completed per policy and to assist in maintaining documentation for supports and services.
Job Qualifications:
- Bachelor's degree in a human service field; such as Sociology, Psychology, Social Work or related field is required.
- Minimum of one-year prior experience working directly with individuals with developmental disabilities and/or related human services program experience preferred.
- Must meet the minimum NYS QIDP requirements.
- Must be able to read, write and speak the English language.
- Valid NYS Driver's License that meets agency standards must be obtained within the first 90 days of employment.
Essential Duties and Responsibilities:
- Habilitative Support: Deliver habilitative supports in small group and individual settings across program and community environments. Teach functional independent living skills tailored to each individual's needs and preferences.
- Program Participation: Actively participate in planning and implementing all Res Habilitation services. Ensure corporate compliance and accurate documentation of all services. Attend planning and treatment meetings and collaborate with team members as needed.
- Skill Development and Personal Care: Provide hands-on assistance with daily living activities, including personal care, medication management, bathing, hygiene, behavior support, meal preparation, housekeeping, laundry, and shopping. Monitor people's physical and behavioral health and report significant changes to supervisors or nursing staff promptly.
- Documentation: Maintain daily service documentation and complete all required records in compliance with OPWDD regulations and agency policies. Monitor DSP completion of assigned responsibilities and documentation. Prepare comprehensive monthly summary notes for assigned people supported.
- Advocacy and Individualized Support: Promote advocacy, individualization, and choice to support each person's outcomes. Respect and encourage peoples' rights, preferences, and relationships, fostering independence and self-directed living.
- Community Life: Develop and implement meaningful in-house and community-based activities emphasizing personal interests, social experiences, and valued roles. Serve as a positive role model in communication, socialization, appearance, and daily living. Facilitate community presences, participation, and inclusion by developing relationships and natural supports that encourage social connections and valued community roles. - Health, Safety, and Hygiene: Ensure a safe, clean, and healthy environment in all aspects of daily living. Support people in meal planning and preparation, promote hygiene practices, and provide training on emergency response procedures.
- Collaboration and Quality Assurance: Review behavioral and medical data for accuracy and completeness. Provide feedback to the interdisciplinary team. Collaborate with clinical staff to assess strengths and needs, implement interventions, and ensure accurate documentation within Medication Administration Records (MAR).
- Additional Duties: Provide on-call support for assigned locations as well as other related duties as assigned by the Residential Manager, Assistant Director or Director.
