Qualification and Competencies

  • High School Diploma and five years' experience in the Human Service field.

Or

  • Bachelor's degree in human service field
  • Demonstrates organizational and communication skills which would enable effective interaction with agency staff, consumers/clients/families and the community.
  • Critical thinking and sound judgment skills are necessary.

 

General Description

Responsible for supportive counseling and providing training in independent living skills.

 

Duties and Responsibilities

  • Participate in developing and evaluating individual service plans, monitor objectives, and document progress monthly or as needed.
  • Train and assist residents with activities of daily living and independent living skills.
  • Monitor medication and promptly notify supervisory personnel of medical, psychiatric, or programmatic emergencies.
  • Provide supportive counseling, behavioral interventions, crisis management, case management, and follow-up services.
  • Accompany residents and provide transportation for recreational and social activities, shopping, medical appointments, and other needs.
  • Assist with day-to-day management of the community residence and oversee operations in the Senior Counselor's absence.
  • Learn and follow all agency policies and procedures and complete other related duties as assigned.

 

Location:

  • Woodmere, NY

 

Schedules:

  • Mondays-Fridays 7am-3pm (full-time)
  • Saturdays and Sundays 3pm-11pm (part-time)
