Minimum Qualifications:
- At least two years of experience in case management or residential care settings.
- Strong knowledge of community resources and social service systems.
- Excellent communication and interpersonal skills to effectively engage with diverse populations.
- Ability to maintain confidentiality and adhere to ethical standards in client care.
Preferred Qualifications:
- Bachelor’s degree in Social Work or a related human services field.
- Master’s degree in Social Work (MSW) or related field.
- Experience working with veterans.
- Experience working with individuals with mental health challenges, substance use disorders, or developmental disabilities.
- Familiarity with electronic case management systems and data entry software.
- Bilingual abilities, particularly in Spanish.
Responsibilities:
- Conduct thorough assessments of residents' physical, emotional, and social needs to develop individualized care plans.
- Coordinate and monitor the delivery of services including healthcare, counseling, and education.
- Advocate on behalf of residents to secure necessary resources and support from community agencies and service providers.
- Maintain detailed and accurate documentation of case activities, progress notes, and service plans in compliance with regulatory standards.
- Provide crisis intervention and support to residents during emergencies or periods of heightened need.
- Facilitate communication and collaboration among residents, families, care teams, and external organizations.
- Participate in regular team meetings and case reviews to evaluate client progress and adjust care plans accordingly.
- Ensure compliance with all relevant policies, procedures, and legal requirements related to residential care.
Skills: The Residential Case Manager utilizes strong assessment and analytical skills daily to identify client needs and develop effective care plans. Communication skills are essential for building trust with residents and collaborating with multidisciplinary teams and external agencies.
