UTHealth Houston Department of Psychiatry & Behavioral Sciences' Texas Child Health Access Through Telemedicine (TCHATT) is hiring a Social Worker (LMSW) to join its team!

This role serves as a care coordinator for TCHATT by managing patients' transitions through the TCHATT system and exercises judgment while coordinating care and referring patients to agencies or health care professionals for other services/transitions in care, as indicated. They also educate the staff on case management and provide specific information related to case types, evaluate patients’ responses and outcomes to education and therapeutic interventions, maintain all documentation in multiple data management systems in a timely manner, and provide education and presentations to community providers and other healthcare providers on community resources and the impact of psychosocial factors on care needs.

This position is currently a remote role, but employees may be required to come into the office periodically for training, meetings, etc. This candidate must be located in the Greater Houston/commutable area in order to work with the local patient population and schools.

Position Summary:

 Independently provides professional social work services to assist patients and their families with connecting to long term care and community resources while patient is receiving mental health services in the TCHATT Program.

Position Key Accountabilities:

  •  Independently provides professional social work services to assist patients and their families with connecting to long term care and community resources while patient is receiving mental health services in the TCHATT Program.

     

  • Plans course of treatment or refers cases to appropriate professionals, institutions or agencies to ensure that patients receive appropriate treatment or referral. Manages patient's transition through the TCHATT system and exercises judgment while coordinating care and referring patients to agencies or health care professionals for other services/transitions in care where indicated.
  • Obtains data from patients and/or relatives in the best interest of their problems to assist in the development of a number of significant teaching endeavors, programs, and social services.
  • With the objective of meeting total patient care, assesses patients and/or relatives to gain understanding of the individual's problems and needs. Collaborates with the physician and other members of the multi-disciplinary team to coordinate care, enhance the patient experience and achieve collaborative goals. Collaboratively develops patient goals, monitors progress, addresses barriers to goal achievement, and evaluates outcomes of individualized care plans. Conducts individual and or group counseling.
  • Prepares detailed reports and may video-tape sessions with patients for review with coworkers or a faculty member.
  • Solicits the assistance of religious, social, and welfare agencies in the treatment of patients. Interacts with social work staff and psychiatric residents.
  • Provides education to physicians, nurses, and other healthcare providers on community resources and the impact of psychosocial factors on care need.
  • Utilizes crisis intervention, conflict resolution, and mediation to address patient/family concerns.
  • Responds to emergency situations.
  • Performs other duties as assigned.


Certification/Skills:

Licensed Baccalaureate Social Worker (LBSW) by State of Texas required

Licensed Master Social Worker (LMSW) by State of Texas preferred.

Bilingual preferred.

Minimum Education:

Bachelor's Degree in Social Work or related field required

Minimum Experience:

1 year of social work experience required Medical social work experience preferred

Physical Requirements:

Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
