 About the Role:

As a Rehabilitation Specialist, you will be a core part of our Psychiatric Rehabilitation Program (PRP), helping individuals with serious mental illness achieve greater independence, resilience, and community integration. Your work will shape the lives of the people we serve — not just through support, but through hope, structure, and meaningful progress.

This position complies with Maryland COMAR 10.63.03.05, which outlines required qualifications and responsibilities for PRP Rehabilitation Specialists.

 What You'll Do:

  • Develop, implement, and monitor Individual Rehabilitation Plans (IRPs) in partnership with each client
  • Deliver structured, goal-oriented rehabilitation interventions in home and community settings
  • Ensure documentation is timely, compliant, and supports medical necessity
  • Provide rehabilitation readiness assessments and strengths-based goal setting
  • Collaborate with mental health clinicians, families, and community supports
  • Lead  and supervise case worker teams and contribute to quality improvement initiatives
  • Maintain compliance with COMAR, HIPAA, Carelon BH guidelines, and CARF standards

 What You’ll Bring:

Required:

  • Bachelor’s degree in a health or human services field (as defined by COMAR 10.63.01.02B(41))
  • Certified Psychiatric Rehabilitation Practitioner (CPRP), LCPC or LCSW-C
  • Minimum of 2 years’ experience working with adults with mental illness
  • Valid Maryland driver’s license, reliable transportation, and clear background check
  • Excellent written and verbal communication skills
  • Ability to use EMR systems and work independently in the field

Preferred:

  • Experience in off-site PRP or mobile mental health services
  • Supervisory experience or demonstrated leadership skills
