About the Role

We are seeking a dedicated and compassionate Rehabilitation Case Manager to oversee the implementation of client treatment plans from admission through discharge and follow-up for our programs in Raleigh, NC. 

In this role, you will serve as the central point of contact for clients, families, healthcare providers, and funders—coordinating therapy schedules, leading interdisciplinary team meetings, and maintaining clinical documentation to ensure high-quality care and seamless treatment progression.

Key Responsibilities & Essential Functions

• Case Management & Goal Alignment: Lead interdisciplinary treatment team meetings to set, monitor, and revise rehabilitation goals. Help the clinical team remain focused on recovery milestones and discharge plans. 
• Client Advocacy & Direct Support: Meet regularly with clients to incorporate their personal goals into care plans. Advocate for client needs and provide guidance across all phases of care. Support direct client care or assist with transportation to medical appointments as needed. 
• Clinical Collaboration: Work alongside the Clinical Director, direct care staff, and external physicians to design strategies that support comprehensive care plans. 
• Family & Stakeholder Engagement: Maintain consistent contact with client families and support networks to deliver updates, education, and orientation. 
• Funder & Financial Management: Monitor care programs according to funding guidelines, facilitate alternative/future funding sources, and complete required documentation to uphold billing integrity. 
• Documentation & Compliance: Maintain thorough, timely, and accurate client charts and log notes in accordance with state regulations, HIPAA/HITECH guidelines, and professional standards. 
• Program Support: Assist with clinical data collection for company-wide quality assurance and outcome initiatives.

Qualifications & Requirements

• Education: Bachelor’s degree in healthcare, human services, or a related field required. Master’s degree preferred. State licensure as a Registered Nurse is preferred.
• Certifications: Certified Rehabilitation Counselor (CRC) or Certified Case Manager (CCM) designation preferred. 
• Experience: Hands-on case management experience within the healthcare industry. Demonstrated ability in crisis management, needs assessment, and treatment plan formulation.
