We are looking for an energetic and enthusiastic person to join our Administrative Team. Community Health Connections (CHC) Health Home is committed to being a transforming and healing presence within our community. Comprised of Care Management Agencies across several counties in the Capital Region and Central New York, the Health Home is committed to serving some of the most needy and vulnerable through care coordination. The ideal candidate will be someone who exhibits compassion and understanding regarding the work the Health Home does to support a complex and vulnerable population.
Position Summary:
As a Lead Health Home, CHC is responsible for ensuring that its subcontracted agencies fulfil their obligations to Health Home Members and conform to New York State Department of Health (NYSDOH) and NYS Office of Health Insurance Programs (OHIP) requirements, as well as other applicable Federal and State requirements. This role would support ongoing Quality Management Program development and execution. The ideal candidate will be someone who exhibits compassion and understanding regarding the work the Health Home does to support a complex and vulnerable population with a myriad of needs, while also recognizing and satisfying the need to comply with evolving Federal and State programmatic requirements. The candidate will possess strong written and oral communication skills, be well organized and exhibit comprehension of Federal and State guidance and regulations and interpretation of that guidance into policies and procedures to inform staff practice. The ideal candidate will have experience in developing audit tools, conducting case and agency audits, interpreting results of audits and putting those results into corrective actions plans to be implemented at the agency level. Additional responsibilities will include oversight of the Quality Team; some travel as this is a regional position and the ability to work in a hybrid environment.
Responsibilities:
- Procure, interpret and operationalize State and Federal guidance and regulations related to the Health Home program.
- Develop and maintain Policies and Procedures manual for Health Home staff based on operationalization of guidance and regulations.
- Develop guidance documents and workflows to help direct care staff understand policies in relation to their day-to-day work.
- Analyze and improve organizational processes to reduce administrative burden for direct care and revenue generating staff.
- Develop tools and methodologies for full Quality Management Program (chart audit tool, interview protocols, surveys, etc.) to ensure quality care is delivered by subcontracted agencies and that policies and procedures are followed.
- Leverage aggregate data to provide meaningful reports to agencies on a regular basis.
- Conduct process and outcome-oriented audits of cases and agency policies and present results in a meaningful and actionable way to subcontracted agencies to ensure quality and promote best practices.
- Serve as the primary point of contact for agencies across counties in both the Capital Region and Central New York for all policy, workflow, and electronic health record questions to ensure quality services are provided.
- Lead monthly meetings with subcontracted agency leadership to discuss health home operations and policies.
- Lead/ participate in Quality Sub-committee meetings with representation from each subcontracted agency to identify and implement changes in areas needing improvement.
- Lead monthly Operations meetings with all agency leadership to discuss operational changes and updates. *Note that at least three times per year, these meetings are held separately and in-person, requiring travel.
- Investigate any incidents (as defined by NYS DOH) and monitor aggregate incident data for trends.
- Supervise a team of Patient Safety and Quality Improvement Specialists.
- Perform supervisory tasks for Patient Safety and Quality Improvement Specialists including but not limited to performance management and timekeeping.
- Work closely with agencies on Performance Improvement Plans to provide guidance and monitor improvement and progress on the plan.
- Train and onboard new subcontracted agencies and continue to support growth of the network.
- Assist in training Samaritan and/or St. Joseph’s Health onsite staff as new staff are hired.
- Provide quality support to the onsite teams at Samaritan and St Joseph’s Hospitals, including audits as needed and facilitation of a monthly quality-focused meeting for each team.
- Engage regularly in strategic planning with Director to ensure continued growth and success of the Health Home.
- Engage regularly in a variety of meetings with NYS DOH to stay informed of programmatic and policy changes and updates.
- Lead/ participate in other internal and external meetings as required.
Education & Experience Requirements:
- HSD/GED with 6 Years Human Services Experience Required.
- Bachelor’s degree in Education, Human Services, Healthcare Administration or a related field preferred.
Pay Range: $35.70 - $51.50
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
