Position Summary

The OPSO Reentry Program Coordinator supports individuals transitioning from incarceration back into the community by coordinating reentry services, addressing immediate and ongoing needs, and connecting clients with appropriate community-based resources. The Reentry Program Coordinator works collaboratively with OPSO case managers, community providers, probation and parole officers, and other stakeholders to promote successful community reintegration, reduce barriers to reentry, and support positive outcomes.

The Reentry Program Coordinator collaborates with OPSO staff, community-based organizations, government agencies, treatment providers, employers, and other partners to help participants obtain the resources needed to reduce barriers and support long-term stability.

Essential Duties and Responsibilities

  • Conduct participant intake interviews and complete required needs, risk, and eligibility assessments. 
  • Develop individualized transition and reentry plans based on each participant’s needs, strengths, goals, anticipated release date, and supervision requirements. 
  • Provide ongoing case management and maintain regular contact with assigned participants. 
  • Help participants obtain identification documents, benefits, transportation, housing, employment, education, treatment, healthcare, and other supportive services. 
  • Coordinate referrals to community-based providers and assist with scheduling appointments when appropriate. 
  • Confirm referral status and follow up with participants and service providers to determine whether services were accessed. 
  • Coordinate with OPSO personnel, probation and parole, courts, treatment providers, workforce agencies, and other community partners. 
  • Identify barriers that may interfere with successful reentry and work with participants and partners to address those barriers. 
  • Document participant contacts, assessments, referrals, services, progress, and outcomes in the designated case-management or data-tracking system. 
  • Maintain accurate, complete, and timely case files in accordance with OPSO policies and grant requirements. 
  • Collect and report program data, including enrollment, services provided, referrals, program completion, release information, follow-up contacts, and participant outcomes. 
  • Participate in multidisciplinary case conferences, staff meetings, trainings, monitoring activities, and program evaluations. 
  • Maintain participant confidentiality and comply with all applicable OPSO policies and legal requirements. 
  • Perform other related duties as assigned. 

Minimum Qualifications

  • Bachelor's degree in social work, criminal justice, psychology, sociology, counseling, public administration, or a related field or extensive relevant experience in case management, reentry services, or a closely related field
  • Relevant case-management, reentry, corrections, behavioral health, workforce development, or social-services experience may be considered in place of educational requirements. 
  • Experience working with justice-involved individuals or populations facing significant barriers is preferred. 
  • Ability to prepare clear case notes, reports, referrals, and other program documentation. 
  • Proficiency with Microsoft Office and the ability to learn electronic case-management and data-reporting systems. 

Knowledge, Skills, and Abilities

  • Knowledge of reentry services, case-management practices, and community resources. 
  • Understanding of common reentry barriers, including housing instability, unemployment, substance use, behavioral health needs, limited education, transportation, and lack of identification. 
  • Strong communication, interviewing, organization, and problem-solving skills. 
  • Ability to establish professional and respectful relationships with participants from diverse backgrounds. 
  • Ability to manage multiple cases, deadlines, referrals, and documentation requirements. 
  • Ability to work independently and collaboratively within a multidisciplinary environment. 
  • Ability to maintain professional boundaries and handle confidential or sensitive information. 
  • Ability to work effectively in a secure correctional setting. 

Working Conditions

The position may require regular work within a secure correctional facility and community-based program locations. The employee must comply with all OPSO security, safety, confidentiality, and facility-access requirements. Some local travel and occasional work outside normal business hours may be required.
