Position Summary
Sanctuary Recovery Centers is seeking a dedicated, organized, and mission-driven Re-Entry Program Coordinator to oversee daily operations at our Vogel supportive housing property and other supportive housing locations as needed.
The Re-Entry Program Coordinator serves as the primary operational lead for assigned housing programs and is responsible for maintaining the day-to-day rhythm of the property, supporting staff performance, monitoring resident compliance, and facilitating access to treatment and supportive services. This role plays a critical part in helping justice-involved individuals achieve stability, recovery, and successful community reintegration through structured housing support and coordinated services. The position also supports program compliance by maintaining accurate records, documenting services in the electronic medical record (EMR), and tracking housing grant and operational data for internal and external reporting.
Essential Duties and Responsibilities
Program Operations
  • Serve as the primary onsite coordinator for assigned supportive housing locations.
  • Ensure safe, organized, and effective day-to-day program operations.
  • Monitor resident adherence to house rules, program expectations, recovery plans, and community standards.
  • Conduct routine property walkthroughs and address operational concerns promptly.
  • Coordinate resident intakes, orientations, room assignments, and transitions.
Resident Support & Care Coordination
  • Assist residents in accessing behavioral health, substance use treatment, medical services, vocational programs, and community resources.
  • Collaborate with case managers, peer support specialists, probation/parole officers, and treatment providers to support resident success.
  • Monitor resident engagement and identify barriers to housing stability, recovery, and employment.
  • Participate in case staffing meetings and interdisciplinary care coordination efforts.
Staff Support & Oversight
  • Provide day-to-day guidance and operational support to housing staff.
  • Assist with scheduling, communication, and accountability processes.
  • Promote a positive, recovery-oriented environment for residents and staff.
Documentation & Compliance
  • Maintain accurate and timely documentation in the electronic medical record (EMR).
  • Complete incident reports, resident notes, housing records, and compliance documentation.
  • Ensure documentation meets AHCCCS, ADHS, grant, and organizational requirements.
  • Track resident outcomes, occupancy data, housing metrics, and program performance indicators.
Reporting & Administrative Functions
  • Maintain housing grant data and reporting requirements.
  • Assist leadership with data collection, audits, inspections, and program reviews.
  • Prepare reports related to resident participation, housing utilization, and program outcomes.
  • Support continuous quality improvement initiatives.
Qualifications
Required
  • Bachelor's degree in Behavioral Health, Social Work, Criminal Justice, Human Services, Business Administration, or a related field; or equivalent combination of education and experience.
  • Minimum of two years of experience working in behavioral health, supportive housing, recovery services, re-entry programs, or case management.
  • Strong organizational, communication, and problem-solving skills.
  • Experience documenting in electronic health record (EHR/EMR) systems.
  • Ability to work independently while collaborating with multidisciplinary teams.
  • Valid Arizona driver's license and reliable transportation.
Preferred
  • Experience working with justice-involved populations.
  • Knowledge of community resources, housing programs, behavioral health systems, and recovery support services.
  • Experience with AHCCCS, ADHS, housing grants, or program compliance reporting.
  • Bilingual English/Spanish.
