Position Summary: The Director of Quality Improvement & Program Performance serves as an operational leader dedicated to embedding continuous improvement, data-driven governance, and regulatory standards across clinical service lines. Working cross-functionally with clinical and operational teams, this role bridges quality frameworks with daily care delivery, driving service optimization, health equity, and high-quality patient outcomes.
Primary Purpose: To manage, implement, and monitor the organization’s clinical, operational, and safety quality initiatives across designated service lines. The Director of Quality Improvement and Program Performance executes the framework for clinical effectiveness, evidence-based practices, and ongoing program evaluation and performance.

Implement Operational Excellence and Process Optimization: Execute continuous improvement initiatives across clinical service lines to streamline workflows, eliminate process waste, and maintain standardized care practices.
Support Performance Management and Lean Process Improvement: Facilitate PDSA cycles and Lean methodologies to support service line quality goals. Assist teams in reducing clinical variation, adhering to standardized workflows, executing improvement plans and corrective actions, and tracking follow-up metrics.
Maintain Clinical Excellence and Evidence-Based Standards: Support the implementation of best-known practices (EBP), fidelity models, and clinical protocols. Monitor compliance with Joint Commission, CARF, and COA standards through regular audits and quality reviews, addressing service access disparities and patient feedback.
Promote Patient Experience, Health Equity, and Engagement: Support a patient-centered culture by gathering patient feedback, helping address care barriers, and incorporating trauma-informed principles into routine program evaluations.
Track Outcomes Framework and Quality Analytics: Collect and analyze quality metrics to support clinical effectiveness and quality payment initiatives. Maintain program performance dashboards and utilize data trends to recommend continuous improvement actions.
Key Performance Indicators (KPI) (Success Measures):
Clinical Path Fid and Standardized Outcome Achievement Rate:
Quality Improvement Plan (QIP) Execution and Sustainability Rate: Achieve a ≥90% on-time completion rate for all milestones within Performance Improvement Plans and Corrective Action Plans (CAPs) within the required timeline, paired with a 0% recurrence rate of the target process failure during the subsequent 12-month monitoring period.
Patient Experience Optimization: Achieve >98% compliance on treatment plans and treatment plan review timelines.
