LOCATION: Bergen Family Center, Englewood NJ (On-site with community/home visit responsibilities)


FLSA STATUS: Exempt


POSITION SUMMARY

The Program Manager of Older Adult Services leads the daily operations of a grant-funded senior wellness initiative focused on reducing social isolation and improving quality of life for older adults. This role oversees care management, participant engagement, group programming, and community partnerships while ensuring alignment with grant requirements and organizational priorities. The position serves as a key driver of service quality, participant retention, and outcome tracking within a person-centered aging services model.

KEY RESPONSIBILITIES

Program Operations / Service Delivery

  • Lead day-to-day implementation of the senior wellness and social isolation reduction program
  • Conduct participant assessments and develop individualized engagement and care plans
  • Provide ongoing care management through home visits, phone outreach, and coordinated service referrals
  • Ensure consistent delivery of group programming, wellness activities, and engagement services in collaboration with program staff
  • Monitor participant engagement, retention, and service utilization trends

Leadership & Supervision

  • Coordinate multidisciplinary staff and program activities to ensure alignment and operational consistency
  • Support training, onboarding, and supervision of volunteers in partnership with program staff
  • Collaborate with Program Coordinator to ensure smooth execution of group activities and events

Compliance & Documentation

  • Ensure completion of baseline and follow-up assessments related to social isolation, emotional well-being, and engagement outcomes
  • Maintain accurate participant records, case notes, and program documentation
  • Track outcomes and prepare reports for internal leadership and external funders
  • Ensure program activities meet all grant, regulatory, and organizational compliance standards

Collaboration & Communication

  • Manage relationships with community partners and contracted service providers
  • Monitor partner performance and service quality, ensuring alignment with program goals and deliverables
  • Collaborate with the Director of Older Adult Services on program planning, reporting, and strategic improvements
  • Communicate regularly with internal teams to support integrated service delivery

Program Evaluation & Continuous Improvement

  • Analyze program data to assess effectiveness and identify trends
  • Identify service gaps, barriers to participation, and opportunities for program enhancement
  • Support continuous improvement initiatives to strengthen participant outcomes and program impact

 

QUALIFICATIONS

  • Bachelor’s degree in social work or related field preferred
  • Minimum of 2 years of experience in program management, care coordination, or aging services
  • Bilingual English/Spanish preferred
  • Valid New Jersey driver’s license and reliable transportation required
  • Must successfully pass all required background checks

  • Experience with case management or client tracking systems preferred


WORK ENVIRONMENT & PHYSICAL DEMANDS

  • Combination of office-based work, community engagement, and home visits
  • Must be able to travel locally for participant visits and partner meetings
  • Ability to lift up to 25 lbs as needed (program materials/supplies)
  • Frequent standing, walking, and community-based fieldwork


WORK SCHEDULE

  • Full-time, 40 hours per week
  • Generally Monday through Friday, standard business hours (9:00 AM – 5:00 PM)
  • Flexibility required for occasional early morning, evening, or weekend programming needs
  • Periodic availability outside standard hours required for urgent program or participant needs


COMPENSATION

  • Salary: $65,000
  • Compensation commensurate with experience and qualifications
