Location: Grand Forks
Shift: Monday-Friday 8am-4pm

JOB SUMMARY:
The purpose of this position is to perform supervised tasks related to the delivery of comprehensive Person-Centered Service Plans (PCSPs) that include health, medication and personal cares, therapeutic, vocational and/or recreational activities for the individuals being served by Anne Carlsen Center in community-based services (Day Habilitation, Employment and/or Autism Waiver Service Management). This position carries specific responsibility in scheduling and direct supervision, guidance, training, coaching, and mentoring to Direct Support Professionals (DSPs), Job coaches, implementing PCSPs, solving issues and needs of the individuals as well as staff. The Program Coordinator develops and promotes positive relationships with the staff as well as the individuals, families, and agencies. The incumbent works closely with the Operations Manager and other ACC Program Coordinators in carrying out efficacious PCSP programs.
QUALIFICATIONS: (minimum requirements)
Education and/or years’ experience required:
- Bachelor’s degree in social work, behavioral science, education or related field
- Two years’ experience working with individuals with disabilities
- DD modules completion or ability to complete within 30 days of hire
- Valid Driver’s License
- Reliable transportation
Preferred Skills and Abilities:
- Three years of experience in supervising, developing, implementing, monitoring, and modifying individual education/habilitation/therapeutic plans
- Two years of experience supporting individuals in developing vocational and employment skills
- Working knowledge of DD Case Management
Supervision of others/decision making aptitude (if applicable): This position would supervise the DSPs and Job coaches
