PRIMARY THERAPIST JOB DESCRIPTION:

 POSITION SUMMARY:     

The Primary Therapist provides quality and organized psychotherapy for identified clients and families, with special emphasis on treatment and discharge planning needs.  The therapist maintains accurate and up-to-date knowledge of all relevant regulations and standards regarding the provision of services and of the implementation of treatment and discharge planning. Professionalism in all aspects of performance must be demonstrated. 

PRIMARY RESPONSIBILITIES & AUTHORITIES:


Interviews clients and families or significant others to obtain social history and data pertaining to treatment and discharge planning.
Demonstrates respect for patient rights by maintaining patient privacy, dignity, safety, confidentiality, and right to refuse or consent to treatment.   
Meets in individual, group or family sessions with clients, families, or both, as requested by Medical Director. 
Completes Psycho-social Assessment and develops therapy section of multi-disciplinary plan of treatment.
Consults with nurse on multi-disciplinary Treatment Plan
Completes all admission paperwork within designed time frame. 
Plans and administers treatment in junction with Medical Director’s diagnosis and Plan of Treatment. 
Organizes and leads structured group sessions of counseling, as well as conducts individual and family therapy sessions as needed per patient treatment plan and Medical Director request/orders.
Updates patient Treatment Plan and adjust treatment modality as needed to better serve patient needs.
Completes therapy notes and documentation by end of each shift unless approved by supervisor to complete the next day. 
Works with other interdisciplinary team members on patient goals, educational needs, social needs, and family relations.
Functions as the Discharge Planner by accurately completing all forms and documents related to discharge, as necessary with input from all members of the staff.
Represents the program at educational meetings as requested by Program Manager.
Works cooperatively with the Program Manager in ensuring that all relevant regulations and standards are met, including Medicare, state licensure requirements, accreditation rules and regulations. 
Collects and compiles data and completes reports as requested by the hospital, Medical Director, Program Director/Manager and Management Company, and as required by the unit’s Quality Assurance Performance Improvement (QAPI) Plan.
Assists in the monitoring and evaluation of psychotherapy, social services, and discharge planning as specified in the QAPI Plan.
Will complete other duties as assigned by Program Director.


PROGRAM DIRECTOR JOB DESCRIPTION:

POSITION SUMMARY:     

The Program Director is responsible for the efficient and successful functioning of the program. These duties can be broken down into four major areas of responsibility: Clinical Management, Unit Management, Contract Management/Hospital Relations, and Utilization Management. The Program Director provides for the implementation of the therapeutic program to meet Hospital Standards, directs and coordinates the activities of staff to ensure quality client care, supervises unit staff, incorporates financial accountability into unit activities, and demonstrates the highest level of professionalism.  

SUPERVISION OF: 

Clinical and Medical aspects of the Outpatient Treatment Program, Nursing, Certified Nursing Aide/Mental Health Technicians and Clerical/Transportation staff.

 

PRIMARY RESPONSIBILITIES & AUTHORITIES:


Assures a therapeutic environment for clients, including client comfort, client rights and client/family education/participation in treatment.
Maintains clients’ daily schedule to provide consistency, stability, and appropriate activities.
Assures client attendance at therapeutic activities.
Evaluates appropriateness and effectiveness of therapeutic activities, modifying these as indicated following consultation with Medical Director, Multidisciplinary Team, and as appropriate the regional director.  
Implements unit admission criteria, in conjunction with Medical Director.
Manages client’s Length of Stay (LOS) to meet multiple goals.                     
Implements Master Interdisciplinary Treatment Planning process and documentation according to unit and accreditation standards.
Utilizes a problem-solving approach.
Coordinates, facilitates communication between/among staff and physicians.
Maintains staffing according to budgeted FTEs.
Conducts monthly staff meetings.
Participates in hospital QAPI process, as required.
Works collaboratively and cooperatively with Medical Director.
Participates in hospital committees, as requested.
Assures compliance with Medicare, state licensure and hospital standards and Accreditation Agencies (HFAP, The Joint Commission, and DNV) requirements.
Assures appropriate appearance, cleanliness, organization and orderliness of unit environment.
Maintains a complaints log and follows up on incident reports according to hospital policy.
Attends Department Head and other meetings, as appropriate.
Coordinates activities with other departments as indicated or requested.
Reports data and information requested to authorize individuals.
Monitors and evaluates staff performance on an ongoing basis to meet job and program standards.
Maintains appropriate employee records.
Recruits and screens potential staff.
Coordinates potential employee interviews with HR Director.
Provides staff counseling, guidance and education to address job descriptions, performance, and identified problems.
Provides regular and ongoing staff education.
Creates an environment conducive to staff growth.
Creates a cohesive group in the unit, which participates in the hospital’s culture.
Implements unit business plan in regard to Average Daily Census on monthly basis and in regard to budget.
Maintains knowledge and awareness of financial and regulatory issues affecting delivery of client care services.
Establish/maintain appropriate patient/family rapport.
Assures referral source follow-up.
Participates in educational activities for unit visibility (unit tours, programs and presentations, etc.).
Represents unit at public events.
Communicates effectively with individuals and with groups about the unit, program and hospital
Maintains expenses within budgetary limits.
Will complete other duties as assigned by the CNO, CEO or medical director.


MINIMUM QUALIFICATIONS (EDUCATION, EXPERIENCE, SKILLS, ABILITIES):


Minimum of a Master’s degree from an accredited college/university. 
KY State License: LPCC or LCSW preferred. An applicant with an associate’s license will be considered if that person is under supervision by a LPCC or LCSW and actively working toward a full license.
Preferred previous program management experience 
Two to three years of clinical experience with older persons preferred.
CPR Certification or will be required to complete CPR certification after hire date. 
CPI Certification will be required after the hire date.
Knowledge of community resources.
Microsoft knowledge/Computer proficiency.
