We provide housing for families, veterans, and low-income seniors. We also support people with developmental disabilities, offer restorative justice programs, and give free HIV testing and education.  VOA started in 1896. JOB SUMMARY AND QUALIFICATIONS:
The PrEP Coordinator (PC) is responsible for coordinating Pre-Exposure Prophylaxis (PrEP) services for HIV-negative individuals and connecting those living with HIV to necessary medical care. The PC will engage in outreach, education, HIV testing, and provide support services while working with community partners to recruit individuals for prevention services. They will link newly diagnosed individuals to medical case management programs and connect eligible individuals to PrEP providers. The PC will work closely with the Program Manager and Senior Prevention Specialist and may also facilitate counseling and group interventions. The role requires strong organizational skills, the ability to work within a collaborative team, and perform data collection and administrative tasks. The PC will work on weekdays with some flexible evening and weekend hours depending on events and must maintain confidentiality while adhering to agency policies and grant stipulations.
 
 
 
WHAT YOU SHOULD HAVE:
  • Bachelor’s degree in social work, human services, public health, or related field OR equivalent experience with a minimum of 2 years direct practice in a similar role or field required.
  • General knowledge of HIV, HIV/STD prevention, PrEP as well as general linkage to care processes preferred. Working knowledge of surrounding populations, field related community partners, and the Jefferson County area.
  • Able to work independently and maintain good organizational and critical thinking skills with fluid verbal and written communication.
  • Previous experience in counseling or group intervention facilitation preferred. Skills in public speaking or education facilitation preferred. Ability to exercise tact, diplomacy and discretion, and to maintain and sustain confidential relationships with clients.
 
 
RESPONSIBILITIES:
  1. PrEP Coordination: Link eligible individuals to PrEP providers and services.
  2. Care Linkage: Connect people living with HIV to medical case management and supported programs.
  3. Collaborate with Outreach Specialists and other community-based partners to identify those with increased risk for HIV.
  4. Conduct HIV including pre and post-test counseling
  5. Promote PrEP awareness and offer PrEP Navigation services to eligible clients.
  6. Data collection to comply with development projects.
  7. Conduct, Log Data, and Report ARTAS and Transform Interventions.
  8. Preform free anonymous and Confidential HIV testing.
  9. Candidate should be comfortable engaging clients in discussion about HIV/STD risks.
  10. Recruit and empower participants to engage in health supporting activities:
          a. Co-facilitate group interventions to support individuals with HIV/AIDS (ex. Healthy Relationships).
                     b. Facilitate one-on-one PrEP counseling for HIV Negative Individuals.
          c. Educate participants on HIV prevention practices and harm reduction techniques.
          d. Assist participants in establishing support networks that promote healthy behaviors.
  1. Distribute safer sex supplies to community partner establishments within Jefferson County.
  2. Perform administrative duties as necessary to support OP efforts.
  3. Assist with community programs/event development and participate in community events that may benefit from HIV/sexual health prevention services
  4. Facilitate/co-facilitate sexual health education classes and other community engagement strategies.
  5. Dedicated two office days for HIV testing.
  6. Record and report all data pertaining to outreach and testing per protocol and established timelines
  7. Document case notes per protocol and established timelines for quarterly case reviews.
  8. Perform quarterly internal reviews of OP files.
  9. Maintain a list of area resources, including stakeholders, speakers and individuals, and groups.
  10. Develop relationships with key partners in community to provide and expand services.
  11. Attend required trainings and continuing education sessions.
  12.  Clear communication with Program Manager and Lead Outreach and Coordinator of all intended activities or support needed
