Salary: $24.95-$25.95 Per Hour

Schedule: Monday-Friday 8am-4pm (40 hr/week) *flexibility with schedule needed to meet programs needs*
Essential Job Functions:
As a Person-Centered Plan Manager, you will be responsible for providing dedicated support to Care Providers in fulfilling their responsibilities. Your key duties will include:
- . Conducting monthly home visits for program members and their Care Providers, ensuring their well-being and compliance with regulations.
- Completing all necessary documentation accurately and promptly to maintain thorough records of each visit.
- Maintaining open lines of communication with assigned staff and coordinating with other service providers, such as day programs and work programs, to ensure comprehensive care for program participants.
- Making appropriate referrals to external resources when additional support is needed.
- Monitoring the home setting according to regulatory guidelines to ensure the safety and comfort of individuals with disabilities.
Qualifications and Skills:
- Must pass background checks and pre-employment screenings including TB test and CORI.
- Bachelor's degree in social work, human services, or a related field (or equivalent experience).
- Basic computer skills using Microsoft Office, email communication, internet, and documentation entry.
- Strong interpersonal and communication skills to effectively collaborate with program participants, their Care Providers, and a multidisciplinary team.
- Ability to prioritize tasks, work independently and within a team, and meet deadlines.
- Compassionate, resourceful, and eager to learn.
- Valid driver's license and reliable personal vehicles meeting state requirements.
