Job position summary:

The Person-Centered Case Manager (PCCM) position is supervised by the Director of Substance Use Treatment and works collaboratively day-to-day with the QMHP of the Substance Use Treatment Department. The PCCM is tasked with; implementing and leveraging strengths-based PCCM to assist in connecting clients with community resources, recovery community and increased daily functioning via the Personal Wellness/Recovery Plan (PWRP). The PCCM will connect these clients to naturally occurring resources in the community, assist in reduction of barriers to fully engage in recovery and provide support in skills development for maintaining a healthy and adaptive recovery lifestyle.

Essential Responsibilities:

  • Articulates all service provisions through timely and quality record documentation to include and not limited to progress notes, third-party collaboration and coordination of treatment with other substance use treatment providers.
  • Connects clients to community resources that support healthy and adaptive recovery lifestyle via the PCCM recovery philosophy.
  • Proactive partner and service provider with the Reno County Recovery Response Team (RRT) responsible for connecting community members, who are or have experienced a relapse or non-fatal overdose the same community and support services the same as with clientele of HMHC who are receiving PCCM.
  • Coordinate services and client care within the organization and substance use treatment clinicians to ensure continuity of care, exceptional quality of care and client-centered services that are within the guidelines of the program definition and customer service expectations.
  • Establish healthy relationships with community resources that are and will be beneficial to support healthy and adaptive recovery for clients.
  • Ensures outcome measure data is accurate and current regarding all program participants regardless of client status.
  • Act in accordance with the established mission, vision, and values.
  • Abide by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).
  • Maintain effective communication and professional working relationships with patients/clients and their representatives, team members, contractors, physicians, peers, outside agencies, and the public.

Minimum Qualifications:

Required Education and Experience

  • Bachelor’s Degree in psychology, social work or a related field.
  • Completion of a Behavioral Health Services (BHS) approved PCCM in Training curriculum

Required License/Certifications/Registrations

  • BHS certification as a Person-Centered Case Manager (PCCM)
  • Valid driver’s license, insurance, and a dependable vehicle

Knowledge, Skills, and Abilities:

  • Knowledge of the substance use provider network and services within the state of Kansas.
  • Understanding of developing and established community resources for support of those in substance use treatment.
  • Knowledge and practice derived from direct service provision.

  • Light Work: Occasionally exerting up to 25 lbs - frequently exerting up to 10 lbs. 11-25% of the day may be standing or walking.
