About the Role

We are seeking a highly organized and detail-oriented Patient Services Coordinator (PSC) to join our team. In this role, you will be responsible for coordinating patient schedules, supporting care continuity, and collaborating closely with clinical teams to ensure efficient service delivery. This position plays a critical role in maintaining high-quality patient care and operational efficiency in a fast-paced healthcare environment.

 

Key Responsibilities

  • Manage and maintain schedules for assigned patients, making timely updates as needed
  • Coordinate clinician schedules, including weekly and weekend coverage
  • Respond to inquiries related to patient and clinician schedules; escalate clinical questions appropriately
  • Schedule admissions, recertifications, resumption visits, and training visits for new clinicians
  • Reschedule missed or declined visits as appropriate to ensure continuity of care
  • Ensure adequate staffing coverage for vacations, call-outs, and holidays
  • Track and maintain accurate patient scheduling details and clinician routes
  • Run daily scheduling and productivity reports; resolve discrepancies and errors
  • Ensure all visits include appropriate visit reasons aligned with insurance and authorization requirements
  • Monitor and maintain compliance with patient visit frequency orders
  • Assist with internal patient transfers between branch locations
  • Collaborate with Clinical Director and Clinical Care Manager to support clinician productivity goals
  • Address and resolve scheduling or care coordination issues identified during after-hours operations
  • Support onboarding by coordinating training schedules for new staff
  • Monitor clinician utilization and report gaps to leadership
  • Provide coverage for other team members as needed
  • Maintain compliance with HIPAA, privacy standards, and company policies
  • Participate in meetings, trainings, and other administrative tasks as required

 

Required Qualifications

  • Bachelor’s degree or equivalent work experience
  • Strong proficiency in Microsoft Office Suite (Outlook, Excel, Word, Teams)
  • Excellent organizational, multitasking, and communication skills
  • Ability to work effectively in a fast-paced, team-oriented environment

 

Preferred Qualifications

  • Degree focus in Social Work, Human Services, Psychology, or Business
  • 1+ year of experience in a home health or healthcare setting

 

Work Environment & Physical Requirements

  • Primarily office-based with frequent sitting and computer use
  • Occasional standing, walking, bending, or lifting (up to 25 lbs.)

 

Travel Requirements

  • Local travel may be required (up to 30%) for meetings, office support, or events
