 Patient Services Coordinator

Join our team and help ensure patients receive the care they need, when they need it.

The Patient Services Coordinator (PSC) is responsible for coordinating patient and clinician schedules, supporting care continuity, and serving as a key communication link between patients, clinicians, and leadership. This role requires strong organizational skills, attention to detail, and the ability to thrive in a fast-paced healthcare environment.

 

Responsibilities

  • Manage patient and clinician schedules to ensure accurate and timely visits.
  • Coordinate admissions, recertifications, and resumption of care visits.
  • Maintain staffing coverage and assist with schedule adjustments.
  • Communicate with patients, clinicians, and leadership regarding scheduling needs.
  • Monitor reports, productivity, and visit compliance.
  • Support onboarding and scheduling of new clinicians.
  • Ensure adherence to HIPAA and company policies.

 

Qualifications

Required:

  • Bachelor's degree or equivalent work experience.
  • Proficiency in Microsoft Outlook, Excel, Word, and Teams.
  • Strong communication, organizational, and multitasking skills.

Preferred:

  • Degree in Human Services, Psychology, Social Work, Business, or related field.
  • 1+ year of home health or healthcare experience.

 

Travel: Local travel as needed (approximately 30%).
