Patient Services Coordinator

The Patient Services Coordinator (PSC) is responsible for coordinating patient and clinician schedules, supporting care continuity, and serving as a key communication link between patients, clinicians, and leadership. This role requires strong organizational skills, attention to detail, and the ability to thrive in a fast-paced healthcare environment.
Responsibilities
- Manage patient and clinician schedules to ensure accurate and timely visits.
- Coordinate admissions, recertifications, and resumption of care visits.
- Maintain staffing coverage and assist with schedule adjustments.
- Communicate with patients, clinicians, and leadership regarding scheduling needs.
- Monitor reports, productivity, and visit compliance.
- Support onboarding and scheduling of new clinicians.
- Ensure adherence to HIPAA and company policies.
Qualifications
Required:
- Bachelor's degree or equivalent work experience.
- Proficiency in Microsoft Outlook, Excel, Word, and Teams.
- Strong communication, organizational, and multitasking skills.
Preferred:
- Degree in Human Services, Psychology, Social Work, Business, or related field.
- 1+ year of home health or healthcare experience.
Travel: Local travel as needed (approximately 30%).
