Patient Navigator, REACCH

Hours: Monday-Friday 8am-4:30pm with 1 evening required until 6:30pm. Monday or Tuesday nights would be the required late evening due to clinic schedule and support group schedules.

Primary Duties – Medical Case Management:

Secondary Duties:
- Provide rapid HIV testing on-site at REACCH and off-site in the community.
- Participate in community outreach & engagement events.
- Provide follow-up with newly diagnosed clients at UPMC hospitals, off-site outpatient offices, and on-site at the REACCH Program, as needed.
- Other duties as assigned.
· Bachelor’s degree in social work, human services, health care or a related field required.
· An ability to demonstrate one’s commitment to social justice strongly preferred.
· Experience providing case management and knowledgeable about community resources in Dauphin County preferred.
· Demonstrated experience exhibiting exceptional organizational, assessment, teamwork, and critical thinking skills.
· Demonstrated ability to motivate and work with people with diverse backgrounds.
· Ability to navigate resources and work independently serving case management clients.
· Experience in the HIV/AIDS field preferred.
· Bilingual in English and Spanish is preferred.
· Understanding of and the ability to apply motivational interviewing, solution-focused, and strengths-based client-focused techniques preferred.
