Patient Navigator- Behavioral Medicine

The Patient Navigator evaluates psychiatric patients in the emergency department and makes recommendations to the emergency department provider as to the disposition of those patients. This position will consult with the attending psychiatrist. This position also evaluates patients throughout the organization as requested or urgently, who are in need of mental health assessments. This position facilitates or recommends appropriate referrals or preventative services, treatment and support for populations with alcohol, drugs and/or substance abuse.

Responsibilities

• Assesses patients for safety risks, mental health conditions, substance use disorders, and psychosocial needs to determine appropriate interventions and level of care in the Emergency Department and throughout the organization as needed.
• Collaborates with multidisciplinary disciplinary team in the Emergency Room and Behavioral Health to develop recommendations regarding patient’s treatment needs.
• Provides crisis intervention, support, and resource information for patients and families experiencing behavioral health emergencies.
• Identifies patients in need of alcohol, drug, and substance use treatment services and provide resources for appropriate treatment programs, community resources, and support services.
• Educates patients and families regarding treatment options, available resources, and recommendations for ongoing care.
• Maintains accurate, timely, and complete documentation in accordance with organizational, professional, regulatory, and legal requirements.
• Ensures compliance with patient rights, confidentiality requirements, safety standards, and applicable state and federal regulations to include Baker Act and Marchman Act.
• Maintains current knowledge of behavioral health resources, community services, and evidence-based practices through ongoing professional development and education.

Qualifications

Minimum Education
 

• Master's Degree Mental Health Counseling, Social Work Required or
• Doctorate Psychology Required

Minimum Work Experience
 

• 2 years Experience working with children over 5, adolescents and adults who have mental health, substance abuse and/or co-occurring disorders or developmental disabilities Required

Licenses and Certifications
 

• BLS for Healthcare Providers (BLS) American Heart Association Upon Hire Required
• Non-violent crisis intervention (CPI) within 90 Days Required
• Ukeru User Certification (UKERU_CERT) within 90 Days Required
