Inova Integrated Behavioral Health is seeking a passionate and experienced Patient Care Coordination Manager to join our dynamic Women’s Health team. This full-time position offers a Monday–Friday day shift schedule (8:00 a.m. – 4:30 p.m.), providing a stable and supportive work environment where you can make a meaningful impact every day.

Within Women's Behavioral Health, the Patient Care Coordination Manager supports individuals experiencing mental health conditions related to reproductive life transitions, including the perinatal period, infertility, pregnancy loss, PMDD, and perimenopause/menopause. As part of an integrated model within the Women's Service Line, collaborates closely with obstetric, gynecologic, reproductive, inpatient, and other interdisciplinary teams to provide evidence-based, trauma-informed, and patient-centered behavioral health care across the continuum of women's health services.

This position is hybrid and will primarily work remotely, with the expectation to attend in-person meetings or appointments at our Fairfax, VA location.

Patient Care Coordination Manager Job Responsibilities:

 

  • Identifies appropriate patient care opportunities for patients and determines patients' eligibility for prevention and diagnostic/clinical treatments. Collaborates with the multidisciplinary team to address patient questions about the care process and treatment options. 
  • Develops individualized patient care for patients seeking treatment by facilitating appointment scheduling, assisting with referral the process and providing information and advocacy to patients throughout care process. 
  • Follows the workflows required by the health information systems used to identify, monitor and document the care management of patients through the entire healthcare continuum including primary, secondary and tertiary care. 
  • Works in partnership with community-based organizations to improve the continuum of care for specific patient populations. 
  • Provides effective and timely referral/transition management support to patients based on individualized needs.

 

Minimum Qualifications:

  • Education: BSW, health, human social services, or related field.
  • Experience: Three years of progressive experience with care or disease management to include coordinating patient care and population health programs.
  • Certification: Basic Life Support Upon Start. 

 

Preferred Qualifications:

  • Strong customer service skills
  • Strong case management/care coordination skills
  • Previous behavioral health experience is preferred but not required
  • Previous women's health experience is preferred but not required
