Job Purpose:
- The Licensed Clinical Social Worker at Rocky Mountain Care Group will provide comprehensive mental health services to individuals and families, focusing on improving their emotional and psychological well-being. This role is critical in delivering high-quality care and support to patients, helping them navigate complex health and social issues within the healthcare setting.
Key Responsibilities:
- Conduct thorough assessments of patients' mental health needs and develop personalized care plans.
- Provide individual, family, and group therapy sessions to address mental health issues and promote emotional resilience.
- Collaborate with healthcare professionals to integrate social work services into patients' overall care plans.
- Advocate for patients' needs and connect them with appropriate community resources and support services.
- Maintain detailed and accurate records of patient interactions, treatment plans, and progress notes in compliance with healthcare regulations.
- Participate in multidisciplinary team meetings to discuss patient care and develop strategies for effective treatment.
- Stay informed about the latest developments in social work practices and mental health therapies to ensure the delivery of evidence-based care.
- Offer crisis intervention and support to patients and families during emergencies.
- Educate patients and their families about mental health conditions and coping strategies.
- Uphold the ethical standards and practices of the social work profession, ensuring confidentiality and respect for all patients.
Qualifications: Required Education:
- Master's degree in Social Work (MSW) from an accredited institution
- Current licensure as a Clinical Social Worker (LCSW) in the state of practice
Required Experience:
- Minimum of 2 years of post-graduate clinical experience in a healthcare setting
- Proven track record of working with diverse populations and managing complex cases
- Experience in providing therapeutic services, including individual and group therapy
Required Skills and Abilities:
- Strong understanding of social work principles, ethics, and best practices
- Excellent communication and interpersonal skills for interacting with patients, families, and healthcare professionals
- Ability to conduct comprehensive assessments and develop effective treatment plans
- Proficiency in electronic health records and documentation processes
- Capacity to work independently and collaboratively within a multidisciplinary team
- Strong problem-solving skills and ability to manage crisis situations effectively
